This is the second part of the trial to add some of our orientation out of clinical practice about the current psychiatric nosology.
In a trial to share in the effort to achieve the essential level of international consensuality by using a common nosological language some sort of compromise was suggested to make use of the enormous and unique effort behind the construction of the ICD-10.
The clinical syndromes as described in the clinical diagnostic guidelines of the ICD-10, would occupy the first diagnostic level.
In a trial to open the gate for added dimensions for optional use, three dimensions are provided (one of which is subdivided into four subscales).
The overlap is expected and notified and the difficulty in agreement is accepted.
Any other possible dimension could be added whether related to colouring of clinical experience.
A very preliminary trial to show the outline of each dimension is given.
The rationale of the proposed multi-dimensional approach with illustrated few examples is demonstrated.
The consequence on high power diagnostic delineation, management, prognosis and research is referred to with an invitation to field trials.

Ceci represente la deuxieme partie dun essai pour contribuer un peu de nos orientations issues de la pratique Clinique a la nosologie psychiat- rique actuelle.
Dans un essai de partager I'dffort en vue daccomplir le niveau essentiei de la consensualite Internationale en utilisant une lan- gue nosotogique commune.
Un compromis de quelque soit la nature est propose pour se servir de l'6norme et unique effort qui est a la base de la construction du ICD-10.
Les syndromes cliniques, decrites dans les direct ifs de diagnostic Clinique de l'ICD-10pourraient occuperle premier niveau diagnostique.
Dans un essai d'ouvrire les portes pour ajouter d autres dimensions a usages facultatifs.
trois dimensions sontpourvues (une d'elles est sous divisee en quatre sous-echelles).
Le chevauche- ment est prevu et notifie.
et la difficult6 de la concordance est admise.
D autre dimension possible pourrait dtre ajout6e qu'dlle ait trait aux dif- ferences culturelles, diffdrents modes de conceptalisation ou couleurs speciales d'expehence Clinique.
Un essai tr6s prdliminaire pour montrer les traits de chaque dimension est fait.
Le rationele de I'approche multi-dimensionelle proposee avec quelques examples illust res est donne.
Ilest ref ere surle retentissement de la delineation diagnostique de haute puissance, la conduite a tenir, le pronostostic et la recherche avec une invitation a faire des essais cliniques.
