الطريقة : أجري هذا البحث على مائة حالة خلال 6 أسابيع إلى 16 أسبوع من الحمل مع حالة إقياء حملي حاد كفئة دراسة بالمقارنة مع مائة حالة حمل محولة للعناية ما قبل الولادة و لا تعاني فئة المراقبة هذه من حالة الإقياء الحاد.
قسمت فئة الدراسة إلى حالة إقياء معتدلة (n=69) و أخرى حادة (n=31) و تميزت الحالة المعتدلة بحدوث الإقياء ثلاث مرات إلا أنها ترافقت مع إحدى الحالات التالية : البيلة الكيتونية, نقص البوتاسمية أو زيادة البولة الدموية.
أما المضاعفات المصاحبة للجنين فهي الاجهاض أو الولادة المبكرة أو نقص في وزن الطفل أو وفاة الجنين داخل الرحم أو تشوه جنيني خطير أو قصور شديد في نمو الجنين داخل الرحم أما بالنسبة للمرأة الحامل فقد تم تقويم الفئتين بما في ذلك الإصابة بمقدمة الإرتعاج و قد تم متابعة الحالات حتى الولادة.
النتائج : كانت نسبة انتشار حدوث الولادة المبكرة و القصور الشديد في نمو الجنين داخل الرحم ونقص وزن الطفل في حالات الاقياء الحاد خلال الحمل أكثر بدرجة كبيرة من الفئة المعتدلة و لم يكن هناك اختلاف إحصائي هام بين الفئتين بما يخص الإجهاض (p=1) و نسبة التقدم الارتجاعي (p=0/78). الخلاصة : نظرا لارتفاع نسبة الولادة المبكرة و نقص وزن المواليد و القصور الحاد خلال الحمل فيجب أن تعتبر هذه الفئة ذات خطورة عالية و من الضروري توفير رعاية خاصة لهن.
Abstract EN
Objective: To evaluate the incidence of some adverse fetal and maternal complications in patients with hyperemesis gravidarum (HEG). Venue: This study is performed in Amir University Hospital, Semnan ,Iran from October 2006 to March 2008. Methods: This study was carried out on one hundred inpatient cases (6 to 16 weeks of pregnancy) with HG as study group and 100 patients who were referred for prenatal care and did not have hyperemesis as control group.
The study group were allocated in mild (n=69) and severe HEG (n=31).
Mild group were patients with more than 3 episodes of vomiting in 24 hours and severe group were patients with the same criteria plus at least one of the following criteria: ketonuria, hypokalemia, increased blood urea.
Fetal outcome include abortion, preterm delivery, birth weight, intra uterine fetal death, major fetal abnormality, intra uterine growth retardation and maternal outcome include preeclampsia in both groups were evaluated.
All patients were followed until delivery. Results: Preterm delivery, Intrauterine growth retardation and low birth weight in severe group was significantly more prevalent than mild and control groups (P<0/001).
There was no statistically significant difference between the study group and the control group in regard to abortion (P =1) and preeclampsia rate (p=0/78). Conclusions: According to high Incidence of preterm delivery, LBW and IUGR in patients with severe HEG, this group of patients should be considered as high risk pregnancy and need to have a special care.
