ردب مكل هو كيس صغير (رأب) يخرج من الجزء الأخير من الأمعاء الدقيقة (المعي اللفائفي) و في بعض الحالات يكون مبطنا بنسيج معدي.
و يعتبر من أشهر الأمراض الخلقية في الجزء الأخير من الأمعاء الدقيقة, و هو عادة لا يسبب مشاكل.
و تظهر الأعراض فقط إذا ما نزف الرأب أو تعرض للانسداد أو التلوث.
الانسداد يمكن أن يسبب ألم مفاجئا شديدا في الجزء العلوي الأيمن من البطن.
و نظرا لأن هذا الألم يتسبب غالبا عن حالات أخرى مثل التهاب الزائدة الدودية و أمراض المرارة, فإن حالة ردب مكل يمكن أن تكون صعبة التشخيص. أفضل اختبار تشخيصي يتضمن حقن صبغة نشطة إشعاعيا إلى مجرى الدم لتسري فيه حتى تصل إلى ردب مكل فتجعله يبدو (مضيئا عند تفحصه بجهاز طبي نووي) جاما كاميرا.
في هذا التقرير نعرض حالة طفل عمره 34 شهرا تم تحويله من عيادة الطوارئ يشكو من نزيف شرجي و تم تشخيصه باستخدام تقنية الأشعة النووية الطبقية الجديدة.
Abstract EN
Meckel’s diverticulum is the most common congenital abnormality of the small gut and a frequent cause of lower GI bleeding.
Early and reliable diagnosis with accurate localisation of the ectopic gastric mucosa as the source of bleeding is essential for prompt treatment.
We report a case of Meckel’s scan in a 34-month-oldboy with sudden onset of rectal bleeding and abdominal pain where a SPECT-CT provided a high-quality diagnostic hybrid image with fusion of function and structure, resulting in rapid and accurate diagnosis and expeditious surgery
