الأهداف : لتقييم التئام العظم، معدل العودة و السيطرة الموضعية على الورم بعد إجراء عملية تجريف الأكياس العظمية الحميدة في الأطراف العلوية عن طريق الجلد عند الأطفال و البالغين.
مكان الدراسة : قسم جراحة العظام و المفاصل في مستشفى الجامعة الأردنية-عمان.
المواد و الطرق : تم علاج 10 من الأطفال و المراهقين (7 ذكورا، 3 إناث) علما أن أعمارهم تتراوح ما بين (3 سنوات و اثنتي عشرة سنة و 3 شهور)، و متوسط أعمارهم 8.57 سنه في الفترة الواقعة ما بين عام 1998 و 2007.
احتوت الدراسة على ثمانية حالات لأكياس عظميه في عظم العضد و حالتين في عظم الكعبرة.
تبين أن 5 حالات كانت عبارة عن كيس عظمي بسيط و 5 حالات كيس عظمي مليء بالأوعية الدموية.
كل الحالات تمت معالجتها من قبل الباحث الأول تحت المخدر العام باستخدام عملية التجريف عن طريق الجلد و بمساعدة جهاز الأشعة ذو الصورة الأكثر حده.
النتائج : تمت متابعة نتيجة العلاج عن طريق التقييم السريري و مراجعة الصور الشعاعيه.
دلت آخر الصور الشعاعيه وجود التئام كامل للأكياس العظمية في كل الحالات التي عولجت في مدة متوسط المتابعة الدورية 2.84 سنوات (المدى ما بين 15 شهر و 4 سنوات).
دلت آخر المتابعات الحديثة للنتائج السريرية عدم وجود أية أعراض مرضيه للأكياس و عودة المرضى إلى كامل النشاطات اليومية.
لم يتم رصد أية مضاعفات أوليه أو متأخرة في أثناء سير المتابعات المستمرة.
الاستنتاج : يعتبر استخدام عملية التجريف عن طريق الجلد في العلاج الجراحي للأكياس العظمية الحميدة في الأطراف العلوية من الجسم علاج فعال للسيطرة على عودة الأكياس و يعزز التماسك العظمي بدون استخدام أية مطعمات عظميه.
Abstract EN
Objectives : to evaluate the bone healing and recurrence rate after percutaneous curettage of bone cysts in the upper extremities in children and young adolescents.
Setting : Orthopaedic department at Jordan University Hospital –Amman.
Methods : The study started from September 1998 to August 2007, treating 10 children (seven males and three females) with a mean age of 8.57 year (range 3 to 12.3 years).
There were eight cysts in the humerus and two in the radius.
The cases comprised 5 simple bone cysts (SBC) and 5 aneurysmal bone cysts (ABC) confirmed histopathologically.
All cases were treated percutaneously under the guidance of image intensifier by the senior author under general anaesthesia.
Results : After a mean duration of follow-up of 2.84 years (range 15m– 4 years), complete healing of the bone cysts had occurred in all cases at the most recent radiograph.
All patients returned to full activities and were asymptomatic.
No early or late complications were encountered during the course of follow-up.
Conclusion : The use of percutaneous curettage in the surgical treatment of benign bone cystic lesions in the upper extremities is an effective treatment to control local recurrences and enhance bony consolidation without using bone graft.
