الأهداف : دراسة نسبة انتشار الأجسام المضادة للدهون الفوسفاتية في المرضى المصابين بتخثر أوعية الساقين و هي ذئبة مانع التخثر (LA)، المضادات للدهون القلبية (aCL)، مضادة البيتا كليكوبروتين (aβ 2-GPI)، و المضادات للفوسفاتيدلسيرن (aPS).
المشاركون و طرق البحث : أجريت هذه الدراسة خلال الفترة من 1/3/2004 و لغاية 1/3/2005 و شملت 50 مريضا مصابا بتخثر الأوعية الدموية للساق و 30 شخصا سليما كمجموعة ضابطة.
تم جمع العينات من المستشفيات التعليمية لمدن الموصل، دهوك، و اربيل في شمال العراق.
المرضى الذين شملتهم الدراسة كانت أعمارهم دون الخمسين سنة و ليست لديهم عوامل خطورة للإصابة بهذه الأزمات.
تم إجراء الدراسة على عينات من البلازم الفحص ذئبة مانع التخثر(LA) و مصل الدم لبقية الفحوصات السيرواوجية و المناعية.
النتائج : كانت نسبة انتشار المضادات للدهون الفوسفاتية هي 42% لدى المرضى المصابين بتخثر الأوعية الدموية للساق و تبين وجود زيادة معنوية لكل من عامل ذئبة مانع التخثر و الأجسام المضادة - ج للمضادات القلبية و البيتا 2 كليكوبروتين.
الاستنتاجات : إن النسبة العالية لمضادات ذئبة مانع التخثر, و المضادات القلبية, و مضادات البيتا 2 كليكوبروتين التي تم قياسها في هذه الدراسة قد تؤكد الدور الفعال لهذه المضادات في حدوث هذه الحالات من تخثر أوعية الساقين.
نستنتج من هذه الدراسة ضرورة إجراء عدة فحوصات للأجسام المضادة الخاصة للدهون الفوسفاتية لربط هذه المضادات مع الحالات السريرية لمتلازمة الدهون الفوسفاتية و للوصول إلى التشخيص الدقيق.
Abstract EN
Objectives : The aim of the study was to determine the prevalence of antiphospholipid autoantibodies (APLAs) among patients with deep vein thrombosis (DVT).
The APLAs tested were lupus anticoagulant (LA), anticardiolipin (aCL), anti-β 2 glycoprotein I dependent (a β 2-GPI), and anti-phosphatidyl serine (aPS).
Subjects and Methods : This study was carried out on 50 patients suffering from DVT events and 30 healthy individuals as a control during the period between 1st March 2004 and 1st March 2005.
The studied cases were under 50 years of age, and had no recognizable risk factors.
The activated partial thromboplastin time (APTT) was used for LA estimation and ELISA assay was used to test for IgG & IgM isotypes of tested APLAs.
Results : The DVT events with significant APLAs was detected in 20 / 50 (40 %) of patients.
A significant titers of LA (P > 0.05), total IgG aCL isotype (P > 0.025), and IgG a β 2-GPI (P > 0.025) were detected in the studied cases, while the correlation was lacking for IgG aPS.
Conclusions : The LA, aCL, and aβ 2- GPI significant titers were found to be predominant in DVT patients.
Therefore, a panel testing for APLAs is a prudent to correlate such DVT cases as a clinical event related to antiphospholipid syndrome (APS).
