تقرير عن حالة ناجحة عند امرأة في التاسعة و العشرين من عمرها ولدت أنثى بصحة جيدة عن طريق العملية القيصرية.
و كانت هذه السيدة قد سبق و أن أدخلت إلى الدائرة عندما كانت في سن السادسة عشر و وجد عندها ورم في المبيض الأيمن.
و بعد إزالة هذه الورم وجد انه سرطاني من النوع الذي يحتوي على خلايا جرثومية (الورم البشري الناسوري) و أثناء العملية وجد أن القشرة المحيطة بالورم كاملة بدون اختراق للورم كما و لم يعثر على آثار انتشار للورم في المبيض الآخر أو في فتحة البريتون.
و بعد المداخلة الجراحية عولجت كيماويا بثلاث عقاقير دون أن تظهر عندها أي خراريج سرطانية لاحقا.
و بعد زواجها نصحت بان تذهب إلى وحدة الخصوبة حيث وجد عندها انسداد في قنوات فلوب عولجت بعدها بدورتين من الإخصاب الخارجي (طفل الأنابيب).
هذا و قد حملت المرأة بعد الدورة الثانية.
و سنناقش في هذا التقرير الطرق الحديثة لعلاج أورام المبيض السرطانية الجرثومية و استعراض تطوراتها المستقبلية بما في ذلك من تأثير على خصوبة المرأة.
Abstract EN
Successful pregnancy in 29-year-old primigravid woman who presented in the department at the age of 16 year with ovarian, malignant, endodermal sinus (germ cell) tumor.
She was operated on and a right salpingo- oophorectomy was performed.
No evidence of local invasion or metastasis to the contra lateral ovary and the tumor had an intact capsule.
Postoperatively, she was treated with three drugs combination chemotheraphy (VAC).
Following her treatment there was no recurrences.
Thirteen years later she became pregnant after two IVF cycles.
We discuss in this report modern management of malignant germ cell tumors, their prognosis and the potential of subsequent resumption of reproductive function in these patients.
