إن انخفاض خلايا الدم البيضاء النيتروفيلي عند الجنة و المواليد هو عبارة عن متناذرة تتشابه في الآلية و الاستمراض لكل من انحلال كريات الدم الحمراء عند الأجنة و المواليد و كذلك انخفاض الصفائح الدموية بينهم على أنه على العكس من مرض انحلال كريات الدم الحمراء من فئة (ريسس) فأن مرض انخفاض خلايا الدم البيضاء في هذه الحالات يحصل عادة في الطفل الأول (البكر).
إن آلية المرض ترجع إلى مستضدات جنينيه لخلايا الدم البيضاء النيتروفيلية التي تدخل إلى الدورة الدموية للأم و لأنها غريبة على مناعة الأم و يرثها الجنين من الأب فأنها تتسبب في استنفار و ظهور أجسام في دم الأم.
و هذه الأجسام المضادة هي من نوع الجلوبيولين المناعي من فئة و الذي يجتاز حاجز المشيمة بسهولة.
و لأن المواليد المصابين بنقص في الخلايا نتيجة انحلالها بواسطة الأجسام المضادة القادمة من الأم فإنهم يكونون عرضة للعدوى البكتيرية و قد تم علاجهم الآن بواسطة إعطائهم الجلوبيولين المناعي عن طريق الوريد بالإضافة إلى المضادات الحيوية.
حديثا فقد تم اكتشاف عوامل نمو دموية بواسطة التقنية الحيوية كالعامل المهيج لتكون خلايا الدم البيضاء الإنساني المركب و الذي يستعمل حاليا في علاج الحالات المستعصية لهذا المرض.
Abstract EN
One of the most dramatic advances of modern medicine has been the discovery and development of diagnostic methods, treatment and prophylaxis of hemolytic disease of the fetus and the newborn (HDFN).
In a few decades, the field has progressed from a complete lack of understanding of the condition to a detailed grasp and understanding of the molecular and immunological bases of the disease.
These advances have in turn culminated in the development of prophylaxis and the near total elimination of its morbidity.
In this review, we shall briefly cover the history and progress of developments of this condition and presents the new concepts in the classification and nomenclature of alloimmune cytopenia of pregnancy.
We will also provide a short account on advances in the pathophysiology, treatment and prevention of this condition.
A review of developments in management will be discussed together with highlights of health care issues of women with alloimmune cytopenia.
