ھذه الدراسة تقارن بین طریقتین للتخدیر العام من حیث استقراریة الجهاز الوعائي و هاتین الطریقتین تستخدمان لغلق الفتحة بین الأذینین من خلال استخدام القسطرة باستعمال سدادة امبلاتزر، تم اختیار ٢٦ مریضا من الفئة ١ و ٢ حسب تصنیف المجمع الأمریكي لأطباء التخدیر و لم یعطي المرضى أي أدویة مھدئة قبل الدخول إلى صالة القسطرة و تم تقسیم المرضى إلى مجموعتین أُعطیت المجموعة الأولى دواء بروبوفول و الالفینتانیل كبدایة للتخدیر و من ثم نفس الأدویة من خلال الدفع البطيء في الورید و أعطیت المجموعة الثانیة الكیتامین و المیدازولوم كبدایة للتخدیر و من ثم نفس الأدویة من خلال الدفع البطيء و لوحظ إن هناك استقراریة للجھاز الوعائي أثناء العملیة للمجموعة الأولى أكثر من المجموعة الثانیة.

Abstract EN

This study compares two anesthetic techniques regarding hemodynamic stability used to anesthetize 26 patients with atrial sepal defect admitted for occlusion of ASD by cardiac catheterization using Amplatzer occluder in Ibn Albita cardiac center in Baghdad, patients are ASA I and II unpremeditated, were divided into two groups, First group was given Propofol and Alfentanil induction then continuous infusion of Propanol and Alfentanil, second group was given ketamine and Midazolam induction then continuous infusion of ketamine and midazolam.

Both groups were given muscle relaxants and intubated with endotracheal tube and ventilated artificially with 100% O2.

The propofok-Alfentanil group showed better hemodynamic stability than ketamine-midazolam group as seen from comparison of HR and BP changes at 5min, 15min, 30 min after induction and at the end of the procedure.

Ketamine in spite of its wide use during cardiac catheterization especially for pediatric patients it has a clear effect on H Rand BP, through its infusion it significantly increases both blood pressure and heart rate as compared to Propofol-Alfentanil infusion which showedmorehaemodynamicstability.
