هذه دراسة مسبقة أجريت في مستشفى الحلة التعليمي العام للمدة من أيلول سنة 2000 إلى أيلول 2005 تشمل الأطفال ذات الأعمار تحت سن الخامسة 32 حالة تم تسجيلها 20 ذكر، 12، أنثى ثلثي الحالات أحيلت من مستشفى الحلة للولادة و الأطفال، 70 بالمائة من الحالات مسبوقة بحالة برد في الأيام القليلة السابقة تم تقسيم الحالات إلى مجموعتين : 1- مجموعة مكونة من الحالات الحادة جدا 2- مجموعة مكونة من الحالات البسيطة و المتوسطة معظم الحالات الحادة تحتاج إلى تداخل سريع مثل وضع أنبوب القصبة الهوائية و بعضها يحتاج إلى عملية فتحة الرغامي بعد ذلك معظم الدراسات بالعالم تؤكد بأن الأطفال في حالات الاختناق يستجيبون للعلاج بواسطة أنبوبة القصبة في دراستنا توجد نسبة عالية من عمليات فتحة الرغامي لعدم وجود وحدات العناية المركزة بصورة صحيحة في مستشفياتنا .
Abstract EN
This a retrospective study was done in Hilla general Teaching Hospital from Sept 2000 to Sept.
2005 for children presented with stridor, age of children below 5 years.
32 cases collected, 20 meals and 12 female, 2 / 3 of them referred from pediatric Hilla general teaching hospital as case of stridor need tracheostomy, 70% of cases have history of flue in last few days, we divided those 32 in to 2 groups: 1.
Those with sever air way obstruction need Immediate establishment of the air way by Intubation or Bronchoscopy then might be tracheostomies. 2.
Those with mild to moderate stridor need to be followed by close absoarvation including time, colure, general condition, Pulse rate (PR), respiratory rate (RR), Temper and Po2.
Most of this group responds to supportive measure like antibiotic, IV fluid, O2, humidified air, steroid.
This medical Treatment cure 23 cases all of the cases.
With sever stridor need intubation (11 cases), tracheostomy done in 7 cases of them.
Most of the studies abroad show that children with stridor can tolerate intubation (Rapid sequence intubation, mostly done by resident Dr's) for long period without complication and the indication of tracheostomy decrease to the minimum.
In our study, the high incidence of tracheostomy due to lack of optimum Pediatric Respiratory Care Unit in our Hospital & lac of good trained staff to deal with endotracheal tube & its complication.
