تم استئصال الزائدة الدودية الوقائي من 56 مريض بعد استحصال موافقتهم أثناء أجراء عملية استئصال المرارة الانتخابية لهـم فـي مستشفيات الحلة الجراحية.
تم مقارنة هذه المجموعة مع مجموعة قياسية شملت 60 مريض.
كلتا المجموعتان كانتا متناظرتان بشكل جيد جدا فيما يتعلق بالعمر، الجنس، و نوع الشق الجراحي.
كل المرضى استلموا جرعة واحدة عن طريق الوريد1 غرام صـوديوم سيفازولين و500 ملم غرام ميترونايدزول اخذت مع بداية التخدير.
أظهرت النتائج إن حدوث الالتهاب التقيحي للجروح ما بعد الجراحة حصل في ثلاثة مرضى (4.5 % من مجموعة حالات استئصال الزائدة الدودية) و في أربعة مرضـى (7.6 % فـي المجموعـة القياسية).
التقيح البطني الداخلي المتبقي لم يجد في أي من المجموعتين.
المدة المتوسطة لإقامة المستشفى ما بعد الجراحة كانت 1.6 يوم (إس دي = 63.1) في مجموعة استئصال الزائدة الدودية و 5.5 يوم (إس دي =85.1) في المجموعة القياسية.
هذه الاختلافات لم تكن هامة بشكل إحصائي.
في ستة حالات من عمليات استئصال الزائدة الدودية (7.10 %) تبين إن الزوائد الدودية التي أزيلت كانت غير طبيعية، بضمن ذلك اثنان مصابات بالالتهاب.
تم الاستنتاج من هذه الدراسة بأن استئصال الزائدة الدودية الوقائي يمكن أن انجازه بسلام أثناء استئصال مرارة انتخابية بشرط أن تعمل بدون تلاعب لا داعي له و المريض محمي بنظام حماية فعال باستخدام المضادات الحيوية الوقائية.
Abstract EN
Prophylactic appendectomy was performed on 56 consenting patients undergoing elective cholecystectomy in hilla surgical hospitals.
The control group consisted of 60 patients.
Both groups were well matched in respect of age, sex, & type of incision.
All patients received a single i.v.
dose of 1g Cefazolin sodium and 500mg Metronidazole at induction of anesthesia.
Postoperative wound sepsis occurred in (3 patients) 5.4% of the appendectomy group and in (4 patients) 6.7% of the control group. Residual intra-abdominal sepsis did not occur in either group.
The mean duration of postoperative hospital stay was 6.1 days (SD = 1.63) in the appendicectomy group and 5.5 days (SD =1.85) in the control group.
These differences were not statistically significant.
Six (10.7%) of the appendices removed were abnormal, including two with inflammation.
The conclusion of this study is that prophylactic appendectomy can be performed safely during elective cholecystectomy provided that it is done without undue manipulation & the patient is protected with an effective prophylactic antibiotic regimen.
