هدفت هذه الدراسة المستقبلية تقييم نتائج طريقة ليمبرج المعدلة في إصلاح ما بعد استئصال الناسور العصعصي.
و قد أجريت الدراسة على أربعون مريضا يعانون من ناسور عصعصي و أجريت لهم الجراحة تحت مخدر نصفي.
تم تقييم النتائج حسب أمد التئام الجرح, مضاعفات ما بعد عملية, طول فترة الإقامة بالمستشفى, و معدل الانتكاس.
بينت نتائج هذه الدراسة أن فترة الإقامة بالمستشفى تراوحت ما بين يومان إلى خمسة أيام (متوسط 2,60 يوما) و أن متوسط عودة المرضى لممارسة حياتهم الطبيعية كانت 10,67 يوما و لا يوجد تنكرز بنسيج العملية و لا يوجد أيضا انتكاس في حالات بعد فترة متابعة متوسطها 8,10 شهرا.
و المضاعفات التي حدثت بعد العملية هي حالتان انفلاق جزئي للجرح و تم علاجهم علاجا تحفظيا, ثلاث حالات تجمع مصلي بالجرح و تم علاجه بواسطة رفع غرزة من الجرح, و حالتان التهابات سطحية بالجروح و استجابوا للعلاج بواسطة المضادات الحيوية.
و أثبتت الدراسة قلة معدل حدوث المضاعفات بعد العملية, قلة الإحساس بالألم بعد العملية, قصر فترة الإقامة بالمستشفى, سرعة العودة إلى الحياة الطبيعية بعد العملية و قلة معدل الانتكاس.
و أكدت الدراسة أن معدل الإصلاح الجراحي بعد استئصال الناسور العصعصي بواسطة طريقة ليمبرج المعدلة هي طريقة فعالة خصوصا في حالات الناسور العصعصي المرتجع و هي طريقة سهلة التعلم و قيمة و تحقق المتطلبات الأساسية لعلاج الناسور العصعصي و هي : الاستئصال الكامل لمنطقة الناسور و الترقيع الأولي بدون شد و بعيدا عن خط المنتصف و التسطيح اللطيف لمنطقة شق الردف.
Abstract EN
Objective : to evaluate the outcome of modified Limb erg transposition flap in surgical reconstruction after excision of pilonidal sinus.
Methods : this prospective study was performed on 40 consecutive patients presented by chronic sacrococcygeal pilonidal sinus, underwent surgical excision and reconstruction with the modified limb erg transposition flap.
Patients were 32 (80 %) males an 8 (20 %) females.
The mean age was 24.13+6.88 (15-48) years.
Thirty four (85 %) patients were presented by primary pilonidal sinus and six (15 %) patients were presented by recurrent pilonidal sinus; two after lay-open method and four after simple primary closure method without lateralization of the scar.
Results : all patients were operated upon under spinal an aesthesia.
The mean length of hospital stays was 2.60 ± 0.86 days (range, 5-2) and the mean time to return to normal activities was 10.67+1.67 days.
There was no flap necrosis or evidence of recurrence has been observed after mean follow-up of 8.10 + 2.29 months (range, 5-12 months).
Reported complications included ; two partial wound dehiscence after removal of sutures, three sarcoma formation, and two patients had developed superficial wound infection.
Conclusions : surgical reconstruction after excision of pilonidal sinus with modified limb erg transposition flap is an effective method for the management of pilonidal disease, especially when dealing with recurrent pilonidal sinus.
It is easily learned and is a valuable technique available to the general surgeon.
