التصلد الجلدي هو احد الأمراض الروماتيزمية المزمنة و الذي يصاحبه تأثر العديد من أجهزة الجسم و نظرا لملاحظة تاثر الجهاز العصبي بما فيه الأعصاب الدماغية لدى المصابين بهذا المرضى فقد قامت هذه الدراسة بهدف استكشاف مدى و نوعية تأثر جهاز الاتزان في هؤلاء المرضى كما استهدف البحث دراسة وجود علاقة بين تأثر جهاز الاتزان و الأعراض الإكلينيكية و التحاليل المعملية للمرضى.
الطريقة : تم فحص 15 مريضة مصابة بمرض التصلد الجلدي و قد تم أخذ التاريخ المرضي بعناية كم تم فحص المرضى إكلينيكيا و تبع ذلك القيام باختبارات السمع الأساسية.
و قد تم عمل اختبارات الاتزان التي اشتملت على اختبار رأرأة العين بالفيديو و الاستجابة العضلية المثارة سمعيا عن طريق عصب الاتزان.
و قد أشتمل اختبار رأرأة العين بالفيديو على الاختبار الكالوري ذو الحرارة المزدوجة لدراسة القناة شبة الدائرية الخارجية و عصب الاتزان العلوي كما أشتمل على اختبار عصب العين الحركي (العصب الثالث) لدراسة مسار الاتزان المركزي.
النتائج : أوضحت النتائج أن 9 مريضات (60 %) كن يعانين من التصلد الجلدي المحدود و 6 (40 %) يعانين من النوع المنتشر.
كان متوسط عمر المرضى 37.56 عام و قد أظهرت النتائج تأثر جهاز الاتزان في هؤلاء المرضى حيث أن 12 مريضة (86.6 %) كن يعانين من تأثر واحد أو أكثر من اختبارات الاتزان، في حين أن اختبار الاستجابة العضلية المثارة سمعيا عن طريق عصب الاتزان أظهر تأثر 12 مريضة (80 %) و الاختبار الكالوري ذو الحرارة المزدوجة ظهر تأثر 2 مريضات (19.9 %).
الخلاصة : أظهرت الدراسة أن اضطراب التوازن في مرض التصلد الجلدي أكثر انتشارا مما كان يعتقد، كما أظهرت أن هذا الاضطراب يميل لأن يكون طرفيا و بالأحرى أن يكون اعتلالا في عصب الاتزان أو اختلالا في الأوعية الدموية.
Abstract EN
Background: systemic sclerosis is a chronic connective tissue disease characterized by multisystem affection.
Nervous system involvement in the form of peripheral and cranial nerve affection has been reported in systemic sclerosis (SSc) patients.
Objective: to study the possible occurrence of vestibular dysfunction in SSc and to determine its nature (peripheral or central).
Also, we aimed to correlate any vestibular involvement with clinical and laboratory data of the patients.
Methodology: Fifteen female patients suffering from SSc were included in this study.
All patients were subjected to careful history taking, clinical examination, basic audiological examination, vestibular test battery in the form of Vestibular Evoked Myogenic Potentials (VEMP) and Videonystagmography (VNG).
VNG included bithermal caloric test to assess the semicircular canal & superior vestibular nerve while the oculomotor test battery was used to assess the central pathway.
Results: Out of the 15 female patients included in this study, nine patients was suffering from limited SSc (60%) while six patients had diffuse SSc (40%).
The mean age was 37.5611.89 years with mean disease duration 8.43 7.3 years.
The results of our study showed a high incidence of vestibular dysfunction in SSc patients where 13 patients (86.66%) showed abnormality in one or more of vestibular tests, 12 patients (80%) having abnormal VEMP results and 3 patients (19.99%) having abnormal caloric test.
Our study showed that Systemic sclerosis has definite vestibular involvement.
The results of VEMP and VNG showed definite peripheral vestibular pattern of dysfunction.
Conclusions: Our results revealed peripheral vestibular dysfunction which is suspected to be due to vascular or inflammatory vestibular neuropathy.
