لمعرفة نسبة نجاح عملية القطع التربيقي الأولي + / - أدوية منع التليف و آثارها على المدى الطويل على العين، تم مراجعة الحالات التي أجريت لها العملية بين يناير 1988 م و ديسمبر 2007م.
المرضى الذين أجريت لهم عمليات سابقة، أو مصابون بالماء الأزرق الخلقي، أو فترة المتابعة أقل من ثلاث سنوات حذفوا من الدراسة.
عرف نجاح العملية بانخفاض الضغط داخل العين ≤ 21 ملم زئبقي.
أجريت الدراسة على 59 عين مصابة بالماء الأزرق الأولي ذو الزاوية المفتوح.
أظهرت الدراسة أن الضغط انخفض مع أو بدون أدوية من 25.1 إلى 16.3 ملم زئبقي في 50 عين (84.8 %) بعد 8.5 سنة متوسط متابعة.
نسبة نجاح العملية لم تزد بصفة مهمة إحصائيا (بي > 0.7225) بعد استعمال أدوية منع التليفات.
العملية فقدت كثيرا من نسبة نجاحها على المدى الطويل، 96.6 %، و 81.2 %، و 75.4 % بعد 5، و 10، و 15 سنة على التعاقب.
هناك علاقة عكسية بين نسبة نجاح العملية و استعمال الأدوية (بي > 0.0012) و عملية ليزر الأرقون التربيقي (بي > 0.0051).
ليس هناك فرقا إحصائيا بين نسبة نجاح العمليات التي تمت بواسطة الأطباء المقيمين، و تلك التي أجريت بواسطة الباحث الرئيسي.
العمليات جميعها غير مصحوبة بمضاعفات خيطرة.
تسعة أعين أظهرت انخفاضا في مستوى نظرها لأسباب عديدة، أهمها تطور الماء الأبيض.
نستنتج أن عملية القطع التربيقي الأولي عملية ذات نسبة نجاح عالية و مأمونة، و مناسبة لتدريب الأطباء المقيمين.
Abstract EN
Long-term efficacy and safety of primary trabeculectomy +/- antimetabolite; assessed by a retrospective study of patients treated at King Abdulaziz University Hospital between, January 1988 and December 2007.
Patients who had previous surgery, congenital glaucoma, or follow up in less than 3 years were excluded.
Success defined as intraocular pressure ≤ 21 mmHg with and without medications.
Fifty-nine eyes of 59 patients, 30 males and 29 females, with a mean age of 61.4 years were reviewed.
Only patients who had primary open angle glaucoma met the inclusion criteria.
Mean intraocular pressure was reduced from 25.1 to 16.3 mmHg, with and without anti glaucoma medications in 50 eyes (84.8%) at a mean follow up of 8.5 years.
Success rate did not improve significantly (p < 0.7225) with antimetabolite.
Life-table showed reduction of success rate over time; 96.6%, 81.2%, and 75.4% at 5, 10 and 15 years, respectively.
There was a reciprocal significant association between success rate and the number of pre-operative topical medications (p < 0.0012), and argon laser trabeculoplasty of p < 0.0051).
Trainee’s success rates were comparable to that of the principle author.
No serious complication was seen.
Nine eyes showed deterioration of vision, mainly due to cataract.
Primary Trabeculectomy is safe, effective, and a suitable training procedure.
