مقدمة : يعد التهاب نتوء المفصل لعظمة العضد من الناحية الوحشية حالة مزمنة و التي تعتبر تحديا في علاجها.
و إلى الآن فإن المراجع التصنيفية التي تبحث في عدد من نتائج علاج هذه الحالة فشلت في استخلاص استنتاجات قاطعة عن العلاج الأكثر فاعلية في علاج مثل هذه الحالات. هدف البحث : تقييم مدى فاعلية العلاج بالليزر ذو المستوى المنخفض و مقارنته بالعلاج بالموجات فوق الصوتية في علاج التهاب نتوء المفصل لعظمة العضد من الناحية الوحشية. طريقة البحث : ستون مريضا بالتهاب نتوء المفصل لعظمة العضد من الناحية الوحشية تم إدخالهم في هذه الدراسة و تم تقسيمهم عشوائيا إلى ثلاث مجموعات : المجموعة (أ) تم علاجهم بالموجات فوق الصوتية مع استخدام رباط دعامي و المجموعة (ب) تم علاجهم بالليزر ذو المستوى المنخفض مع استخدام رباط دعامي ثم المجموعة (ج) و هي المجموعة الضابطة تم علاجهم باستخدام رباط دعامي فقط.
تم قياس درجة الألم و مدى مجال حركة مفصل الرسغ و قوة قبضة اليد سواء القصوى أو غير المصحوبة بألم عند بداية الدراسة.
و تم قياس مدى تحسن المريض و استفادته من العلاج مع كل القياسات الأخرى بعد أربعة ثم ثمانية أسابيع من العلاج و أيضا بعد أربعة أسابيع من انتهاء العلاج. النتائج : أسفرت هذه الدراسة عن انخفاض ذو دلالة إحصائية في درجة الألم لكل مجموعات المرضى المشتركين في الدراسة مع وجود فارق ذو دلالة إحصائية في درجة الألم بالنسبة للمرضى الذين تم علاجهم بالموجات فوق الصوتية أو بالليزر ذو المستوى المنخفض عن المرضى في المجموعة الضابطة، و أيضا بين المرضى الذين تم علاجهم بالموجات فوق الصوتية و أولئك الذين تم علاجهم بالليزر ذو المستوى المنخفض. الاستنتاج : تقدم هذه الدراسة دليلا آخر على مدى فاعلية الليزر ذو المستوى المنخفض و الموجات فوق الصوتية كعلاج لحالات التهاب نتوء المفصل لعظمة العضد من الناحية الوحشية مع أفضلية للموجات فوق الصوتية.
Abstract EN
Background: Lateral epicondylitis (LE) is a chronic condition that has proven to be challenging to treat.
To date, systematic reviews looking at a range of outcome measures on treatments for LE have failed to draw any firm conclusions as to what treatment is most effective in managing this condition. Objective: To evaluate the effects of Low-level Laser therapy (LLLT) and to compare these with the effects of US treatment in the management of tennis elbow. Methodology: Sixty patients with LE were enrolled in this study and randomized into three groups: group A treated with US plus brace, group B treated with laser plus brace and group C (control group) used brace only.
Pain, grade of tenderness, range of wrist motion and both maximal and pain-free grip strength were assessed at baseline.
Patient's perception of benefit through a global improvement, and all other outcome measures were assessed at four, eight weeks of treatment and four weeks after the end of treatment. Results: Significant reduction in pain from baseline in all studied groups with significant difference between mean VAS of pain of patients treated with either US or laser and that of patients in control group were detected in all stages of the study.
Furthermore, significant difference in pain reduction between US treated group and laser treated group.
Also significant improvements in all other outcome measures were detected. Conclusions: The current clinical study provides further evidence of the efficacy of LLLT and pulsed low US in the management of LE with better results obtained using US.
