في هذه الدراسة نحاول التمييز بين مرض التهاب المفاصل الروماتويدي و التهاب المفاصل المصاحب للالتهاب الكبدي الفيروسي (سي) بإستخدام أداة جديدة أكثر خصوصية لالتهاب المفاصل الروماتويدي مثل الأجسام المضادة للكيراتين.
و ذلك بانتقاء 50 مريضاً إيجابيا لعامل الروماتويد و قد قسم المرضى إلى مجموعتين : المجموعة الأولى : 25 مريضا مصابون بمرض الالتهاب الكبدي (سي).
المجموعة الثانية : 25 مريضا يستوفون الخصائص التي وضعتها جمعية الزمالة الأمريكية للروماتيزم لتشخيص مرض التهاب المفاصل الروماتويدي.
و قد تم إجراء التحاليل الآتية : صورة دم كاملة-وظائف كبد-سرعة الترسيب-بروتين سي التفاعلي- الأجسام المضادة لفيروس (سي)-الأجسام المضادة للنواة-بالإضافة إلى الأجسام المضادة للكيراتين.
و قد تم انتقاء 15 شخصا أصحاء كمجموعة ضابطة لا يظهرون أي نتائج إيجابية لأي من التحاليل السابقة.
كما تم عمل أشعة عادية على اليدين لكل المرضى مع استبعاد من وجد به دلالة على تآكل العظام.
و عند الكشف سيرولوجيا عن وجود الأجسام المضادة للكيراتين وجد أنها تظهر بنسبة كبيرة في المجموعة الثانية (60 %) و بنسبة أقل في المجموعة الأولى (12 %) و لا تظهر في المجموعة الضابطة.
كما أظهرت النتائج أن خصوصية التشخيص بواسطة الأجسام المضادة للكيراتين كانت بنسبة 88 % بينما كانت حساسية التشخيص بنسبة 60 %.
و قد أظهرت هذه الدراسة أهمية و خصوصية الأجسام المضادة للكيراتين في تشخيص التهاب المفاصل الروماتويدي و كذلك في التفرقة بينه و بين التهاب المفاصل المصاحب للالتهاب الكبدي الفيروسي (سي).
Abstract EN
Background: Among the different clinical disorders associated with hepatitis C virus (HCV) infection, articular involvement is a frequent complication.
Symmetric polyarthritis associated with HCV infection frequently displays a rheumatoid arthritis (RA) like clinical picture.
Thus, differentiating patients with HCV related symmetric polyarthritis from patients with RA represents both a diagnostic and a therapeutic challenge. Objective: Our aim was to investigate whether antikeratin antibodies (AKAs) could be useful markers in differentiating patients with rheumatoid arthritis (RA); from patients with hepatitis C virus (HCV) associated polyarthritis who are seropositive for rheumatoid factor (RF). Methodology: Serum AKAs were determined in two groups of patients; all were RF seropositive.
Group (I): 25 patients with HCV associated symmetric polyarthralgia or arthritis; Group (II): 25 patients with RA (ACR revised criteria for diagnosis of RA, 1988).
Fifteen healthy individuals, age and sex matched, served as controls.
AKAs were tested using an indirect immunofluorescence technique with 1:10 serum dilution.
Results: The study revealed that AKAs were detected in 15/25 (60%) patients with true RA and in only 3/25 (12%) patients with HCV associated arthritis (p<0.0001).
AKAs were not found in the sera of the healthy controls.
Conclusions: AKAs can be useful markers in differentiating patients with true RA from those with HCV arthritis.
