أجرى هذا البحث لمعرفة فاعلية الحقن بالبوتكس لعلاج حالات قصر وتر أكليس عند الأطفال في بدايات مراحل المشي الأول بدلا من الجراحة مع برنامج علاج طبيعي و تأهيلي متزامن.
وقد أجرى هذا البحث على أربعين طفلا من سن سنة و شهرين إلى سنتين و سبعة أشهر بمتوسط ستة و سبعة أشهر، وقد قسمت الأطفال إلى مجموعتين، كل مجموعة تضم عشرين طفلا.
المجموعة الأولى : تضم الأطفال ذوي الشد "القصر" البسيط وقد قسمت إلى مجموعتين كل مجموعة تضم عشرة أطفال (أ، ب)، المجموعة أ تم حقنها بالبوتكس أ مع برنامج التأهيل الخاص و المجموعة ب لم يتم حقنها و استمرت بالعلاج و البرنامج التأهيلي الخاص.
المجموعة الثانية : تضم الأطفال ذوي الشد من النوع المتوسط وقد قسمت إلى مجموعتين كل مجموعة تضم عشرة أطفال (ج، د)، المجموعة ج تم حقنها بالبوتكس مرتين مرة كل 3 أشهر مع برنامج التأهيل الخاص، بينما المجموعة د لم يتم حقنها و استمرت بالعلاج و البرنامج التأهيلي المتخصص فقط.
وقد أظهرت النتائج تحسنا ملحوظا للأطفال الذين تم حقنهم بالبوتكس عن اللذين لم يتم حقنهم بالبوتكس في سرعة الاستجابة للبرنامج التأهيلي و التحسين السريع دون أعراض جانبية.
وقد خلص البحث إلى التوصية، باتباع الحقن بالبوتكس كوسيلة علاجية مساعدة لتلك الحالات للتقليل من الحاجة إلى العمليات الجراحية لتطويل وتر أكيلس.
Abstract EN
Objective: To evaluate the therapeutic effect of intramuscular injection of Botox A on children with mild to moderate shortening of tendoachillis.
Methodology: Forty children were involved in this study, the age ranged from 1 & 2/12 year and 2 & 7/12 years with mean age 1 & 7/12 ± 4/12 years.
Patients were divided into two equal groups.
Patients of the first group had mild tightness, and subdivided into 2groups A & B, each included 10 cases.
Group A was treated by Botox injection, while group B received other physical modalities without Botox injection.
The second group had moderate tightness and subdivided into two groups (C & D), each included 10 cases.
Group C received two Botox injections (once every 3 months) while group D received other physical modalities.
Each group was monthly evaluated up to 6 months, by modified Ashworth scale, patients’ global response Btx A treatment, ankle ROM, pain score, and walking time. Results: There was high percentage of improvement in groups A and C in pain score, kinetic score, tenderness, and walking score.
The percentage of improvement was less in group B and D. Conclusions: Botox A intramuscular injection in the calf muscles of children with mild to moderate tightness tendoachillis had good effects to decreasing pain, tenderness, and muscle tone.
