الهدف : قياس مستوى الكروتيزول و الباراثرمون في الأطفال المصابين بمرض الرثيان المفصلي مع تحديد علاقتهما بكل من العوامل الإكلينيكية لنشاط المرض و الإنترليوكن -1 ب و الإنترليوكن -6. الطريقة : أجريت هذه الدراسة على ستة و ثلاثين مريضا بمرض الرثيان المفصلي و ثمانية عشرة أصحاء كمجموعة ضابطة. و قد تم قياس مستوى الكورتيزول و الباراثرمون و الإنترليوكن -1 ب، -6 في كل من المرضى و المجموعة الضابطة، أيضا تم فحص المرضى إكلينيكيا و إجراء الفحوصات المعملية لهم. النتائج : أوضحت النتائج أن مستوى الكروتيزول و البارثرمون كان منخفضا بدلالة إحصائية عالية بين المرضى مقارنة بالمجموعة الضابطة. أما الإنترليوكن -1 ب، -6 فكان مرتفعا بدلالة إحصائية عالية في المرضى خاصة في المجموعة ذات زيادة في النشاط المرضي مقارنة بالمجموعة الضابطة. كما وجدت علاقة عكسية ذات دلالة إحصائية عند مقارنة مستوى الكروتيزول و الباراثرمون مع الإنترليوكن -1 ب، -6 و أيضا مع العوامل الإكلينيكية لنشاط المرض. الاستخلاص : في مرض الرثيان المفصلي في الأطفال يقل مستوى الكورتيزول و الباراثرمون خاصة في المجموعة ذات زيادة في النشاط المرضي، و بما أن ذلك ذو علاقة مع العوامل الإكلينيكية فقد يساعد هذا في تقويم تقدم و توقف المرض.
Abstract EN
Objective: This study was designed to define the levels of cortisol and parathormone in children with juvenile rheumatoid arthritis (JRA) in relation to disease activity as well as to the levels of the proinflammatory cytokines interleukin-1 ? (IL-1 ? ) and interleukin-6 (IL-6).
Methodology: Thirty six patients (26 females and 10 males; aged 6-12 years; mean 7.3 ± 1.6 yr) as well as 18 apparently healthy controls were recruited into the study.
Patients and controls were subjected to clinical examination and a comprehensive battery of investigations including a complete blood picture, ESR, CRP, serum levels of calcium, phosphorus, alkaline phosphatase, IL-I ? , IL-6 as well as cortisol and parathyroid hormone (PTH).
Results: Serum levels of IL-1?? and IL-6 were significantly higher in JRA patients than those of the controls (p<0.001).
This rise was more evident in patients with an active disease.
On the other hand, serum levels of both cortisol and PTH were significantly lower in patients compared to the controls (p<0.001).
Moreover, there were significant negative correlations between serum levels of both cortisol and PTH with IL-1 ? & IL-6 as well as other parameters of disease activity (ESR & CRP) (p<0.05 & p<0.001, respectively).
There was also a significant negative correlation between PTH only and serum calcium (p<0.05).
Conclusions: Decreased serum values of both cortisol and PTH were demonstrated in the children suffering from JRA, especially those with a high disease activity.
This hormonal status may indicate a disturbed homeostasis contributing to a more active disease.
