الهدف من الدراسة : هو تقييم العلاقة بين رصفة عظمة الرضفة و الألم و الوظيفة في الفصال العظمي بالركبة. مادة البحث : تم اختيار 60 مريض يعانون من مرض الفصال العظمي بالركبة و قد تم فحصهم إكلينيكيا و قياس مؤشر كتلة الجسم و عمل اختبارات نفسية لقياس الاكتئاب باستخدام مقياس (CES-D).
كما تم قياس الألم و الوظيفة بالركبة بمؤشر (WOMAC) الذي يتكون من معامل مسطرة الألم و المعامل الوظيفي.
و خضع المرضى للفحوصات المعملية لقياس صورة الدم و سرعة الترسيب و حمض البوليك بالدم و تحليل بول.
أما بالنسبة للفحوصات التصويرية فقد تم عمل أشعة سينية عادية في وضع الوقوف للركبة منظر أمامي خلفي و جانبي و منظر فوقي لتحديد مقياس كليجران و لورانس، كما تم التصوير بالرنين المغناطيسي (لتحديد رصف عظمة الرضفة) على الركبة في وضع عمودي لعمل بعض القياسات لتحديد طول عظمة الرضفة و طول عظمة البكرة و النسبة بينهما (ناتج قسمة طول عظمة الرضفة على طول عظمة البكرة)، و أيضا تم عمل تصوير بالرنين المغناطيسي في وضع أفقي لقياس مؤشرات عظمة البكرة (عمق البكرة و درجة ميل عظمة البكرة و زاوية عظمة البكرة) و قياس مؤشرات عظمة الرضفة. النتائج : وجد أن هناك علاقة ذو دلالة إحصائية بين طول عظمة الرضفة و درجة ميل عظمة البكرة مع مقياس الألم.
و لم تكن هناك علاقة ذات دلالة إحصائية لزاوية عظمة البكرة و زاوية الأخدود في عظمة البكرة مع قياس الألم كما وجد علاقة لكنها لم تصل لدلالة إحصائية بين زيادة درجة ميل عظمة الرضفة و نقص نقطة الإنقسام الثنائية و قياس ألم الركبة.
كما وجد أن هناك اتجاه للزيادة في درجة ميل عظمة البكرة و وظيفة الركبة لكنها لم تصل إلى درجة ذات دلالة إحصائية.
و لم تكن هنالك علاقة ذو دلالة إحصائية مع باقي المؤشرات و وظيفة الركبة. الخلاصة : يعتبر الفصال العظمي بالرضفة جزءا منفصلا عن الفصال العظمي بالجزء الأيمن و الأيسر بين عظمتي القصبة و الفخذ، و قد وجد أن هنالك علاقة ذات دلالة إحصائية بين بعض قياسات رصف عظمة الرضفة و الألم و الوظيفة في الفصال العظمي بالركبة، و لذلك إذا اتخذت الإجراءات المناسبة لتحسين ميكانيكية عظمة الرضفة و تآكل الغضاريف قد يكون مفيدا في علاج الألم الناتج عن الفصال العظمي بالرضفة.
Abstract EN
Objective : To find out the correlation of patellar alignment with pain and function in knee osteoarthritis (KOA).
Methodology : Sixty (KOA) patients were subjects for clinical assessment; body mass index, psychometric assessment using Center of Epidemiologic Studies Depression Scale (CES-D).
We assessed knee pain and function using the Western Ontario and McMaster Universities (WOMAC) index.
CBC, ESR, serum uric acid and urine analysis were performed.
Postero-anterior plain x-rays in weight bearing were done for radiological grading of (KOA) using Kellgren-Lawrence score.
Magnetic resonance image (MRI) was done in a sagital plane to measure patellar length (PL) and trochlear length (TL).
Patellar length ratio (PLR) was calculated by dividing PL by TL.
MRI in a transverse plane was used to measure trochlear indices (depth SA, inclination LTI and angle TA) and patellar indices (LPTA and BO).
Results : PLR and LTI increased significantly with WOMAC pain, not so with TA or SA.
There was an increase of LPTA and a decrease of BO with WOMAC pain but not to a significant level.
There was a tendency of LTI to increase with WOMAC function.
PLR increased with WOMAC function but not to a statistical level.
TA, SA, LPTA and BO showed no significant change with WOMAC function.
Conclusions : PF arthritis should be considered as an entity separate from tibio-femoral arthritis.
There was an association between patellar alignment with knee pain and function.
Therefore, procedures to improve patellar biomechanics to correct articular damage may be beneficial to alleviate pain.
