A comparative study on the outcomes of in tracytoplasmic sperm injection between women with polycystic ovary syndrome and those with sonographic polycystic ovary-only

Objective : this study was performed to compare the outcomes of intracytoplasmic sperm injection between women with PCOS and those with sonographer PCO-only Design : Case-control study.

Setting: Assisted Reproduction Technology Unit, International Islamic Center for Population Studies & Research, Al-Azhar University.

Methods : this was a prospective study of women with PCOS (n = 55) and those with Sonographer PCO-only (n = 53).The control group consisted of 100 infertile women with normal ovaries, which was matched in terms of the age and BMI with the former two groups.

All patients in the three groups had male factor infertility necessitating ICSI using fresh ejaculated spermatozoa.

Interventions : controlled ovarian hyper stimulation and ICSI.

Main Outcome Measures : Oocyte number, fertilization rate, clinical pregnancy rate, Miscarriage rate and ovarian hyper stimulation syndrome (OHSS).

Results : despite a significantly lower total gonadotropin dose, a significantly higher serum E2 level was attained in both the PCOS and the PCO-only groups compared to the control group.

Although significantly more oocytes were retrieved and more embryos were available in the PCOS and PCO-only groups, than the control group.

No significant differences were noted in the clinical pregnancy (32 % 32 % and 34.6 % respectively) and Miscarriage (17.6 %, 18.7 % and 17.6 % respectively) rates among the three groups.

However Severe OHSS rates were significantly higher in women with PCOS (14.5 %) and PCO-only (13.2 %) compared to those with normal ovaries (2 %).

Coasting was .used significantly more in the first two groups (7.2 % and 5.6 % respectively) than the control group (0%).

Conclusion: sonographer PCO only women are not less responsive to COH as compared to women with PCOS; considering similar pregnancy outcomes and incidence of adverse events.

During COH for IVF or ICSI, careful monitoring is necessary in women with sonographer PCO only as in those with PCOS.
