الهدف : هناك بيانات محدودة حول تقييم وظائف الكلى في مرضى الثلاسيميا الكبرى، و لا توجد دراسة من هذا القبيل لدى المرضى العمانين.
تهدف هذه الدراسة إلى إيضاح نمط وظائف الكبيبات و أنابيب الترشيح الكلوية باستخدام فحوصات مختبرية مختلفة لوظائف الكلى للمرضى المصابين بالثلاسيميا الكبرى.
الطريقة : شملت هذه الدراسة المقطعية للفترة (يناير-يوليو 2008) على 30 مريضا مصابا بالثلاسيميا الكبرى، الذين يعتمدون على نقل الدم (15 أنثى و 15 ذكر تراوحت أعمارهم بين 32-16 سنة، متوسط ± انحراف معياري 21.23 ± 3.42 سنة) و الذين يراجعون عيادة الثلاسيميا بالمستشفى السلطاني، سلطنة عمان.
تمت مراجعة السجلات الطبية و المختبرية من ضمنها وظائف الكلى والتي شملت على : الكرياتينين و اليوريا و الفوسفات لمصل الدم، و معدل الترشيح الكبيبي، و نسبه طرح الصوديوم المترشح، و منسب الألبومين إلى الكرياتينين بالبول، و منسب البيتا 2 مايكروكلوبيلين إلى الكرياتينين بالبول، نسبة امتصاص الفوسفات في أنابيب الترشح الكلوية، و الامصتاص الأقصى للفوسفات في أنابيب الترشيح إلى معدل الترشيح الكبيبي.
النتائج : كان معدل الترشيح الكبيبي لدى جميع المرضى > 90 مليليتر / دقيقة / 1.74 متر2، و كرياتينين مصل الدم > 90 مايكرومول / لتر، و اليوريا < 6ميليمول / لتر، و منسب الألبومين إلى الكرياتينين بالبول < 2.5 ملغم / ميليمول.
كانت نسبة طرح الصوديوم المترشح > 1 % لدى اثنين من المرضى (6.7 %) فقط و كان منسب البيتا 2 مايكروكلوبيلين إلى الكرياتينين بالبول > 22 مايكروجرام / مليمول عند ثلاثة مرضى (10 %) فقط.
كان لدى جميع المرضى نسبة امتصاص الفوسفات في أنابيبب الترشيح الكلوية > 0.85 بضمنهم سبعة مرضى (23.3 %) فقط كانت القيم لديهم بين 0.85 و 0.95، و كذلك كانت القيم > 0.95 لدى 23 مريضا (76.7 %).
كان لدى جميع المرضى منسب الامتصاص الاقصى للفوسفات في أنابيب الترشيح إلى معدل الترشيح الكبيبي > 1.0 مليمول / لتر من ضمنهم مريض واحد (3.3 %) فقط كانت لديه النتيجة ما بين 1.0-1.5 مليمول / لتر، و 29 مريضا (96.7 %) كانت النتائج لديهم أكثر من 1.5 مليمول / لتر.
كان مستوى فوسفات مصل الدم > 1.4 مليمول / لتر عند 24 مريضا (80 %).
احصائيا بين فوسفات مصل الدم و منسب الامتصاص الأقصى للفوسفات في أنابيب الترشيح إلى معدل الترشيح الكبيبي (r = 0.904, P < 0.001).
الخلاصة : يبدو أن نتائج وظائف الكبيبات و أنابيب الترشيح الكلورية لدى المرضى المصابين بالثلاسيميا الكبرى تبقى ضمن المجال الطبيعي و لا تتأثر بالمرض.
لوحظ ارتفاع نتائج نسبة الامتصاص الأقصى للفوسفات في أنابيب الترشيح إلى معدل الترشيح الكبيبي و كذلك نسبة امتصاص الفوسفات في أنابيب الترشيح الكلوية عند غالبية المرضى نتيجة زيادة فوسفات مصل الدم و ما يترتب على ذلك من ارتفاع امتصاص الفوسفات في أنابيب الترشيح الكلوية.
Abstract EN
Objectives: There are limited data concerning the assessment of renal function in beta-thalassaemia major, with no study of such involvement in Omani patients.
The objective of this study was to establish the pattern of renal glomerular and tubular function using traditional and specific laboratory tests in patients with beta-thalassaemia major.
Methods: This cross-sectional study, from January–July 2008, included 30 patients of the Thalassaemia Clinic at the Royal Hospital, Oman, with transfusion-dependent homozygous beta-thalassaemia major.
They included 15 males and 15 females, aged 16-32 years with mean ± standard deviation of 21.23 ± 3.42 years.
The medical records were reviewed and renal function states assessed as follows: serum creatinine, estimated glomerular filtration rate (eGFR); urea; phosphate, fractional excretion of filtered sodium (FENa); urine albumin: creatinine index; urine ß2-microglobulin:creatinine index; tubular reabsorption of phosphate (TRP), and tubular maximum phosphate reabsorption (TmP)/GFR.
Results: All patients had eGFR >90 ml/min/1.73m2; serum creatinine <90 μmol/L; serum urea <6.0 mmol/L, and urine albumin:creatinine <2.5 mg/mmol.
Only 2 (6.7%) patients had FENa >1% and 3 (10.0%) patients had urine ß2-microglobulin: creatinine >22 μg/mmol.
All patients had TRP >0.85, of whom seven (23.3%) patients had values within the range of 0.85-0.95 and 23 (76.7%) had values >0.95.
Also, all patients had TmP/GFR >1.0 mmol/L, of whom only one (3.3%) patient had TmP/GFR of 1.0–1.5, and 29 (96.7%) patients had TmP/GFR >1.5 mmol/L.
Finally, 24 (80%) patients had serum phosphate >1.4 mmol/L.
Linear regression revealed a highly significant correlation between serum phosphate and TmP/GFR (r = 0.904, P < 0.001).
Conclusion: Renal function, glomerular and tubular, appears to be well preserved in beta-thalassaemia major.
Almost all renal function indicators were within the recommended ranges.
Raised TmP/GFR and TRP were noted in the majority of patients, reflecting an up-trend in serum phosphate and therefore increasing renal phosphate reabsorption.
