الهدف : قياس تركيز الأستيوبنتين في البلازما في مرضى الذئبة الحمراء و قياس مدى علاقته بالناحية الإكلينيكية والمعملية و كذلك علاقته بنشاط المرض وعلاقته بالتغيرات الباثولوجية.
الطريقة : و قد أجريت هذه الدراسة على 20 مريض بمرض الذئبة الحمراء و مذبك 10 أصحاء كمجموعة ضابطة و تم قياس تركيز الأستيو بنتين لجميع المرضى و كذلك المجموعة الظابطة و تم أخذ عينة من الكلى و فحصها تحت الميكروسكوب في خلال 3 أشهر في وجود بروتين في البول أو دم في البول.
النتائج : و قد أظهرت النتائج زيادة في تركيز الأستيوبنتين في البلازما ذو دلالة إحصائية فى مرضى الذئبة الحمراء عنه فى المجموعة الضابطة و كذلك وجود علاقة بين مستوى الأستيوبنتين ونشاط المربض.
كما أظهرت النتائج زيادة فى تركيز الأستيوبنتين فى البلازما ذو دلالة إحصائية في مرضى الذئبة الحمراء و لديهم إصابة الكلى عنه في باقى المرضى.
الاستنتاج : و قد خرج البحث بأن الأستيوبنتين قد يشترك بجزء فى إصابة الكلى في مرض الذئبة الحمراء.
Abstract EN
Aim : to measure plasma concentration of osteopontin and to correlate these levels with clinical, laboratory, disease activity and histopathologically parameters in SLE patients.
Methodology : this study was performed on 20 SLE patients and 10 normal control subjects.
Plasma osteopontin concentrations were measured by ELISA technique for both patients and control groups.
All patients underwent renal biopsies within 3 months of onset of proteinuria or hematuria.
Results : there was a highly significant difference between patients and control groups as regards plasma osteopontin concentration (p < 0.001).
There was a significant positive correlation between osteopontin and SLEDAI (p < 0.001) and activity index of renal biopsies (p < 0.05).
Also there was a highly significant differences as regard osteopontin plasma concentration between patients with and without renal affection (p < 0.001) Conclusion : osteopontin has been shown at least partly to account for SLE nephritis probably through predominance of Th1-type response in both peripheral and renal tissue.
Recommendations : Further investigation of this mechanism in lupus nephritis may allow the design of new therapeutic strategies of lupus nephritis such as manipulation of Th1 / Th2 and down-regulation of Th1-response.
