أهداف البحث : دراسة للاستئصال الجزئي للملفوفة (القرينة) السفلي تحت الغشاء المخاطي و هو يعتبر من الطرق المسجلة للحصول على مجرى هوائي أنفي مفتوح في حالات تضخم الملفوفة السفلي.
طرق البحث : تم عمل دراسة على 50 مريض بتضخم الملفوف السفلي بإجراء جراحة استئصال جزئي تحت الغشاء المخاطي للملفوفة السفلي بالمناظير الضوئية و جهاز ال Microdebrider و تم تقييم المرضى بعد العملية من حيث حالة الملفوفة السفلي قبل و بعد الجراحة و الشعور بانسداد الأنف قبل و بعد الجراحة و تم رصد المضاعفات التي حدثت لهم و تمت متابعة هذه الحالات بعد ثمانية أسابيع من الجراحة.
النتائج : من ميزات جهاز ال Microdebrider مع المناظير الضوئية في إجراء هذه الجراحات التحكم في حجم و مكان الأنسجة المراد استئصالها.
و قد تم تسجيل عمليتين فقط من نزيف ما بعد العملية من 90 عملية (2.2 %) و خمس حالات عانت من الجفاف الطفيف و تكوين قشرة بالأنف.
في حين لم يسجل أية حالة من تكون رائحة كريهة بالأنف أو ضمور في أغشية الملفوفة السفلي بسبب استئصال زائد.
الاستنتاج : النتائج تشير أن الاستئصال الجزئي تحت الغشاء المخاطي للملفوفة (القرينة) السفلي بالمناظير الضوئية و جهاز ال Microdebrider هو طريقة آمنة للحصول على تقليص في حجم الملفوفة مع مضاعفات قليلة.
Abstract EN
Background / Aim : Sub mucous resection of the inferior turbinate's is a recorded surgical technique for reducing their size to achieve patent nasal airways in situations where an enlarged turbinate's cause chronic nasal airway obstruction.
Many other techniques and complications have been applied and evaluated in the literature.
We studied endoscopic assisted micro debrided sub mucous inferior turbinate reduction as a technique performed with endoscopy and micro debride and assessed its success and complication rates.
Study Design : A prospective study of 50 adult patients who underwent endoscopic sub mucous resection of the inferior turbinate's with a micro debrided.
Patients and methods : Patient questionnaires were used for subjective assessment of symptoms before and after the procedure.
Each patient's inferior turbinate's was graded for size from I to EI before and 8 weeks after surgery for objective analysis.
Results : The advantage of the micro debrided technique with endoscopy is the precise control of the amount and location of tissue that is removed from the sub mucosal plane.
The complications encountered with this technique are limited to postoperative bleeding that occurred in 2.2 % of turbinate's.
There was dryness and crusting in 5 patients and no post-operative foul odor or excessive removal of tissue.
Conclusion : The results show that endoscopic sub mucous resection of inferior turbinate's with micro debrided is a safe method of achieving turbinate size reduction with minimal morbidity.
