الهدف : تحديد وظائف الغدة الدرقية و الأجسام المضادة في مرض الذئبة الحمراء. طريقة البحث : أجريت هذه الدراسة على 35 مريضا بالذئبة الحمراء و 10 أشخاص أصحاء كمجموعة ضابطة.
و أجريت الاختبارات الإكلينيكية و المعملية المعتادة للمرضى و الأصحاء.
و كذلك قيست هرمونات الغدة الدرقية، التراي أيودوثيرونين الكلي التتر أيودوثيرونين الحر و الهرمون المنبه للغدة الدرقية.
كما تم قياس الأجسام المضادة لكل من الثيروجلووبيولين الميكروزوم. و تم تقسيم المرضى إلى ثلاث مجموعات تبعا لنتائج اختبارات الغدة الدرقية : انخفاض نشاط الغدة الدرقية تحت الإكلينيكي (10 مريضا)، زيادة نشاط الغدة الدرقية الإكلينيكي (8 مرضى) و نشاط الغدة الدرقية الطبيعي (17 مريضا). و عندما تم إجراء اختبارات t بين المجموعة الضابطة و كل من مجموعات المرضى وجدنا أن المجموعة الثالثة أظهرت انخفاضا في نسبة الهيموجلوبين و زيادة في معدل سرعة الترسيب و زيادة في نسبة الكرياتينين في المصل و لكنها ذات دلالة إحصائية عالية، كما وجدنا أن الهرمون المنبه للغدة الدرقية (1.13 ± 2.4) ذو دلالة إحصائية عن المجموعة الضابطة (0.52 ± 1.72). و من ناحية أخرى وجدنا زيادة في مستوى كل من معدل سرعة الترسيب و الكرياتينين في المصل و التتر أيودوثيرونين الحر في المجموعة الثانية عن مستوياتها في المجموعة الضابطة و كانت ذات دلالة إحصائية عالية.
بين كان هناك انخفاض في مستوى عدد كرات الدم الحمراء و الهيموجلوبين عن المجموعة الضابطة و كان ذات دلالة إحصائية عالية. أما في المجموعة الأولى كان هناك انخفاضا في مستوى كل من الهيموجلوبين و التتراأيودوثيرونين الحر عن المجموعة الضابطة بينما كانت هناك زيادة في مستوى كل من معدل سرعة الترسيب و الكرياتتينين في المصل و الهرمون المنبه للغدة الدرقية عن المجموعة الضابطة و كانت كلها ذات أهمية إحصائية عالية. ثلاثين في المائة من المرضى أظهروا نتائج إيجابية للأجسام المضادة للميكروزوم و عشرون في المائة (20%) أظهروا نتائج إيجابية للأجسام المضادة للثيروجلوبيولين و وجود هذه الأجسام المضادة أظهر علاقة إيجابية مع الهرمون المنبه للغدة الدرقية. الاستنتاج : أن هناك تغيرات في وظائف الغدة الدرقية في مرضى الذئبة الحمراء و تتناسب مع نشاط المرضى و بالأخص إصابة الكلى. التوصيات : و ننصح بعمل وظائف الغدة الدرقية و الأجسام المضادة في مرض الذئبة كجزء من التحاليل المطلوبة للاكتشاف المبكر لاختلال الغدة الدرقية في مرضى الذئبة الحمراء.
Abstract EN
Objective: to determine the patterns of thyroid dysfunction and autoantibodies associated with systemic lupus erythematous (SLE).
Methodology : the study was performed on a group of 35 SLE patients and 10 matched apparently healthy subjects as a control group.
Various clinical and laboratory parameters of SLE were measured for both groups.
Also thyroid function tests were measured, which included TT3, FT4 and TSH.
Ant microsomal and ant thyroglobulin antibodies were measured for patients.
Patients were classified according to their thyroid function tests into 3 subgroups: subclinical hypothyroid (10 patients), subclinical hyperthyroid (8 patients) and thyroid (17 patients).
Results : we found that the thyroid subgroup showed lower hemoglobin level, higher ESR, and higher serum cretonne level that was of a high statistical significance (p < 0.01).
Also TSH level (2.4 ± 1.13) was higher than controls (1.72 ± 0.52) and this was of statistical significance (p < 0.05).
In the hyperthyroid group, the level of the ESR, serum cretonne and FT4 was higher than that of controls.
This was also of a high statistical significance (p < 0.01).
The level of HB and RBCs was lower than in controls with high statistical significance.
In the hypothyroid group, The levels of Hb and FT4 was lower than in controls while the level of ESR, serum cretonne and TSH was higher than controls with high statistical significance (p < 0.01).
Thirty percent (30 %) of the patients showed positive ant microsomal antibodies and twenty percent (20 %) showed positive ant thyroglobulin antibodies.
The presence of these antibodies correlated positively with TSH levels.
This might explain the thyroid disorders in SLE patients.
Conclusion : performance of thyroid dysfunction tests in SLE patients, as part of biochemical and immunological profiles, may help in early detection of associated thyroid disorders.
