أثبتت الدراسات الحديثة أن هناك علاقة بين البكتريا الحلزونية و الإصابة بتليف الكبد و أورام الكبد و لذلك كان لزاما أن نكشف عـن هذه العلاقة في المرضى المصريين المصابين بالتهابات مزمنة نتيجة الفيروس الكبدي سي و أورام الكبد الأولية.
تمت الدراسة على العينات الأرشيفية الموجودة في قسم الباثولوجي بالمعهد القومي للأبحاث الأمراض المتوطنة و الكبد و قسمت الـى مجموعتين : -المجموعة الأولى : بلوكات الشمع المحضرة من أنسجة المرضى المصابين بالتهاب فيروسي كبدي مزمن (سي) و عـددهم 45 مريض منهم 31 رجلا و 14 سيده تتراوح أعمارهم بين 27 الى 57 سنه.
-المجموعة الثانية: بلوكات الشمع المحضرة من أنسجة المرضى المصابين بأورام سرطانيه أوليه في الكبـد و عـددهم 15 مريض منهم 11 رجلا و 4( سيدات) تتراوح أعمارهم بين 49-78 سنه و كلهم مصابين بفيروس سي.
تم عمل فحص ميكروسكوبي للعينات المصبوغة بصبغات الهيماتوكسلين و الأيوسين و كذلك تم عمل الصبغة النسيجية المناعية على عينات الكبد المأخوذة من هؤلاء المرضى و أثبتت النتائج أن 33 مريضا من إجمالي 45 (نـسبه 73.3 %) المـصابين بالتهاب فيروسي مزمن سي كان لديهم ميكروب البكتريا الحلزونية في النسيج الكبدي و عدد 5 من إجمالي 15 (نسبه 33.3 %) المـصابين بأورام في الكبد كان لديهم البكتريا الحلزونية في عينات الكبد.
و من هذه النتائج يتضح وجود البكتريا الحلزونية في عينات النسيج الكبدي المأخوذة من المرضى المصريين المـصابين بـأمراض الكبد المزمنة نتيجة فيروس سي و الأورام مما يستوجب إجراء دراسات مستقبليه لإثبات ما إذا كان لهذه البكتريا الحلزونية دور في تطور مراحل المرض وشدته كعامل مساعد مع الفيروس (سي).
Abstract EN
Background / Aim : recent studies have suggested that bacterial confection with Helicobacter species in patients already infected with hepatitis C virus (HCV) could be involved in the development of cirrhosis and hepatocellular carcinoma (HCC).
A retrospective study was performed in order to explore the association between Helicobacter pylori and HCV in hepatic tissue of Egyptian patients with chronic hepatitis C and hepatocellular carcinoma.
Methods : the presence of Helicobacter pylori was tested by immunohistochemistry on liver samples from two groups of patients ; chronic hepatitis C (group I, n = 45) and hepatocellular carcinoma (group II, n = 15) Results : Group I (chronic hepatitis C) involved 31 males and 14 females (male / female, 2.2:1).
Their age ranged from 27 to 58n years with a mean of 45.8 ± 7.0 years and HCC group involved 11 males and 4 females (male / female, 2.8:1).
Their age ranged from 48 to 78 years with a mean of 60.2 ± 10.0 years.
Immunostaining revealed H.
pylori microorganisms in 33 / 45 biopsies (73.3 %) of chronic hepatitis C group and 5 / 15 (33.3 %) of HCC group (p = 0.005).
Conclusion : H.
pylori organisms were present in liver tissues of HCV and HCC patients with significantly higher proportion in the former.
Further studies are needed to ascertain its possible role, if ever, in the pathogenesis of cirrhosis and hepatic malignancy in Egyptian patients.
