تمت هذه الدراسة على 50 مريضا (28 رجلا و 22 سيدة) تراوحت أعمارهم بين 21 – 75 عاما يشكون من انتفاخ مزمن بالبطن مصحوبا بهزال و وجود دم بالبراز و أنيميا مع اشتباه وجود أورام بالبطن و خاصة في بعض الحالات التي لها تاريخ مرضي مثل إجراء استئصال أورام بأي من أعضاء الجسم.
و قد تم فحص جميع المرضى بالعيادات الخارجية لاختيار الحالات التي يتشبه وجود أورام بها لاستكمال الفحوص الإشعاعية. و قد تبين من الدراسة دقة الأشعة المقطعية التقديرية متعددة المقاطع في تشخيص أورام القولون و المستقيم بنسبة عالية تصل إلى 100% حيث تم تشخيص 23 حالة كأورام خبيثة من الحالات التي يشتبه أن بها أورام غير حميدة و تمت مقارنة النتائج مع نتيجة الفحص بمنظار القولون التقليدي و تبين تطابق التشخيص مع المنظار و مع نتائج الجراحة في 18 حالة و أما الخمس حالات الأخرى فقد تأكدت بالفحص الباثولوجي. و تم أيضا تشخيص 18 حالة كأورام حميدة (زوائد لحمية بالقولون) باستخدام منظار القولون في حين تم تشخيص 12 حالة بالأشعة المقطعية التقديرية كأورام حميدة (زوائد لحمية بالقولون)، و تبين وجود 9 حالات أورام خارج القولون كأورام ثانوية. و تبين من الدراسة إمكان تشخيص و تحديد درجة الورم الخبيث من حيث امتداده للأعضاء المحيطة بالقولون و أعضاء الجسم بصفة عامة حيث تم تحديد 57,14% من الدرجة الأولى و الثانية و 87,5% من الدرجة الثالثة و 66,6% من الدرجة الرابعة. و لذا ننصح باستخدام الأشعة المقطعية التقديرية متعددة المقاطع في تشخيص الحالات التي يشتبه بوجود أورام بالقولون أو المستقيم نظرا لدقتها العالية و أيضا إمكان تحديد درجة انتشار الورم للأنسجة المجاورة للقولون و أعضاء الجسم الأخرى و كذلك إضافة المنظار التقليدي للقولون كمكمل للفحص و إمكان أخذ عينة من الورم إذا تبين ذلك للطبيب أثناء الفحص.
Abstract EN
The study was conducted to evaluate the role of MDCT colonography in diagnosis of colo-rectal neoplasm’s compared to conventional colonoscopy.
Fifty patients were included in the present study 28 males and 22 females with age ranged from 21-75 years old.
Patients were presented with variable-size large bowel masses examined by MDCT colonography and conventional colonoscopy.
Twenty three cases were diagnosed by CT colonography as colonic malignancy, 18 cases diagnosed by colonoscopy as colorectal benign polyps "8 polyps measured less than 5mm, 6 polyps from 5- 10mm and 4 more than 10mm", those were correlated with findings of CT colonography which revealed only 12 benign polyps with size ranged from 6mm to more than 10 mm.
The other 9 cases proved to be extra colonic metastases.
Conclusion : MDCT colonography allows more definitive evaluation of the different colo-rectal and extra-colonic masses as it is a non-invasive modality.
It has a higher sensitivity for detecting a clinically suspected colonic neoplastic mass.
The conventional colonoscopy may still have an advantage of detecting very small and superficial mucosal lesions and taking biopsy from the suspected superficial areas.
