يعتبر مفصل الكتف من أكثر المفاصل التي تتعرض لعدم الثبات أو الخلع و هي تحدث بنسبة 1-2 % في سن مبكرة تتراوح من 15-35.
تم هذا البحث على 43 مريض يشكون آلاما مزمنة أو عدم ثبات المفصل، و قد تم عمل رنين مغناطيسي و موجات فوق صوتية لجميع الحالات و تبين من الفحص وجود أسباب عديدة لتلك الآلام و لعدم ثبات المفصل منها الالتهابات الحادة أو المزمنة للأوتار أو العضلات و وجود ارتشاح بالمفصل أو وجود تمزق بالعضلات أو كسر سواء في رأس عظمة الذراع أو تجويف مفصل الكتف نتيجة حادث أو الضغط المفاجئ على العضلات كما في الرياضيين كلاعبي التنس أو الرجبي أو ألعاب اليد.
و قد تبين من البحث دقة الرنين المغناطيسي في تشخيص هذه الحالات من حيث إظهار العمق الحقيقي للمفصل و جميع العضلات و الأوتار المحيطة به أو ما قد يتعذر تشخيصه بوسائل التشخيص الأخرى و قد تصل نسبة دقة التشخيص بالرنين مع الصبغة للمفصل إلى 95 %، و كذلك الموجات فوق الصوتية عالية التباين تساعد إلى حد كبير في تشخيص أي قطع بالأوتار أو عضلات الكتف و تشخيص الالتهابات بالمفصل بنسبة تصل إلى 90 %.
لذا ننصح بالبدء في تشخيص هذه الحالات باستخدام الموجات فوق الصوتية عالية التباين نظرا لسرعة التشخيص به و تقبلها من معظم المرضى و قلة تكلفتها المادية و كذلك أدانها في مختلفة الأوضاع الحركية للمفصل، و في حالة تعذر التشخيص أو الشك في سبب المرض أو عدم وضوحه ننصح باللجوء للرنين المغناطيسي و بخاصة مع الصبغة للمفصل على تشخيص عالي الدقة.
Abstract EN
Shoulder pain and dislocation usually occur more frequent in young athletes, this study was carried out to assess the role of MRI & US in evaluation of chronic shoulder pain and instability of the shoulder joint.
43 patients complaining of chronic shoulder pain and some of them complaining shoulder instability were examined by MRI, MR orthography and high resolution ultrasonography.
All patients had anterior glen humeral dislocation (41 males, 95.3% and 2 females, 4.7%).
On MR orthography, the antero-inferior labrum was torn in 35 case "81.4%", the anterior labrum was torn in 4 cases "9.3%" and attenuated in 4 cases "9.3%", while on conventional MRI the percentages were 44.3% (19 case), 4.7% (2 cases) and 4.7% (2 cases) respectively.
US depicted full anterior labral tear in 39 of 43 patients.
Sensitivity was 92%, and accuracy was 92%.
MR orthography has been proved to increase the sensitivity and accuracy of MRI for detecting labro-ligamentous complex and rotator cuff tendon abnormalities.
Accuracy of US in depiction of labral tears and associated fractures indicates that US can provide a useful information in patients with anterior shoulder instability.
