المقدمة توجد الخلايا الجذعية في كل الأنسجة ذاتية التجدد حيث أن لها خصائص مميزة.
ففي بعض الحالات المرضية يتم تدمير الخلايا الجذعية إما جزئيا أو كليا مما يؤدي إلى تغطية القرنية بخلايا الملتحمة مع ظهور الأوعية الدموية بالقرنية.
الهدف من البحث هو تقييم دور زرع الخلايا الجذعية المستمدة من نفس المريض في أعادة بناء سطح العين.
طريقة البحث : شملت الدراسة اثنان و أربعون عينا لمرضى تم اختيارهم بصورة عشوائية و يعانون من نقص الخلايا الجذعية سواء الجزئي أو الكلي في عين واحدة و قد تم إجراء جراحية زرع الخلايا الجذعية المستمدة من نفس المريض.
النتائج اشتملت الدراسة على ثمانية عشر من الذكور و أربعة و عشرون من الإناث يتراوح أعمارهم بين 19-72 سنة بمتوسط عمري 46.8 ± 14.4 سنة.
شملت التدخل الجراحي سرطان الملتحمة المؤثر على الخلايا الطلائية فقط، الظفرة الأولية و المرتجعة، أمراض القرنية الناتجة عن العدسات اللاصقة و الحروق كيميائية.
كان متوسط حدة الإبصار المصححة قبل الجراحة 0.49 ± 0.29 و ما بعد الجراحة كانت 0.74 ± 0.21، و قد كان الفارق بين النتائج ذو دلالة إحصائية عالية.
الخلاصة : إن زراعة الخلايا الجذعية المستمدة من نفس المريض تعتبر إجراءا ناجحا في علاج بعض أمراض العيون المصحوبة بنقص جزئي أو كلي في الخلايا الجذعية حيث يمكن استخدام الخلايا الجذعية من نفس العين أو العين الأخرى، كما إن زراعة الخلايا الجذعية المسمدة من نفس المريض تعتبر إجراءا آمنا ذو مضاعفات قليلة.
Abstract EN
Introduction : stem cells are present in all self-renewing tissues and have unique properties.
In certain pathological conditions, the limbal stem cells may be destroyed partially or completely resulting in varying degrees of stem cells deficiency with conjunctivalization of the cornea with vascularization.
Objective : to evaluate the outcome of autologous limbal stem cell transplantation in ocular surface reconstruction.
Material and Methods : Forty two randomly chosen eyes with partial or complete unilateral limbal deficiency treated by autologous limbal stem cell transplantation.
Results : There were 18 males and 24 females with mean age 46.8± 14.4 years.
The indications for intervention included Bowen's disease, primary and recurrent pterygium, contact lens induced keratopathy, persistent epithelial defect and chemical injury.
The mean preoperative best corrected visual acuity was 0.49 ± 0.29.
While the mean postoperative best corrected visual acuity was 0.74 ± 0.21.There was a highly statistically significant difference between the results.
Conclusion : autologous limbal cell transplantation is an effective procedure in reconstructing of the ocular surface for ocular disorders associated with partial or diffuse limbal stem cell deficiency as it could be carried out from the same or contra lateral healthy eye.
It is a safe technique with minimal complications.
