هدف البحث: تحديد مستويات بعض الإنترلوكينات مع البروتين التفاعلي C بالإضافة إلى نسبة إيجابية الإنتانات بجراثيم الملويات البوابية HP و الفيروسات المضخمة للخلايا CMV في حالات التهاب الكولون غير المحدد في مدينة بغداد.
طرق البحث: أجريت هذه الدراسة المقطعية المستعرضة في مشفى الكندي التعليمي في بغداد، شملت مجموعة الحالات 60 مريضا أعمارهم بين 19 سنة راجعوا المشفى خلال الفترة بين كانون الثاني 2008 و حتى الشهر نفسه من عام 2010 .
أما مجموعة الشاهد فقد تكونت من 30 من المتطوعين الأصحاء الموافقين لمجموعة المرضى من حيث العمر و الجنس.
تم الحصول على عينات مصلية لتحديد مستويات الإنترلوكين 10- (IL-10)، العامل المنخر للورم ألفا (TNF-α)، مستوى البروتين التفاعلي C و أضداد جراثيم الملويات البوابية IgG.
كما تم تقييم المرضى لوجود إنتان بالفيروس المضخم للخلايا من خلال الاختبارات المصلية (الأضداد IgG و IgM).
النتائج: بلغ متوسط مستوى الإنترلوكين 10 في حالات التهاب الكولون غير المحدد (15.2 ± 39.3 بيكوغرام / مل) و هو أعلى و بشكل هام إحصائيا بالمقارنة مع مجموعة الشاهد (قيمة 0.024=P، بفواصل ثقة 95 %، 21.87-1.57).
أما مستوى العامل المنخر للورم ألفا TNF-α فقد بلغ (9.04 ± 22.36 بيكوغرام / مل) في حالات الكولون غير المحدد، و هو مستوى أخفض و بشكل هام إحصائيا بالمقارنة مع مستواه لدى مجموعة الشاهد (0.796 ± 33.31 بيكوغرام / مل) (قيمة 0.000=P، بفواصل ثقة 95 %، 13.3- % و حتى 8.6-).
أما نسب البروتين التفاعلي C، أضداد جراثيم الملويات البوابية IgG و أضداد الفيروسات المضخمة للخلايا IgM فلم تلاحظ فيها فروقات هامة إحصائيا بين المجموعتين.
الاستنتاجات: تشير هذه النتائج لأهمية الإنترلوكين 10- و العامل المنخر للورم ألفا في مراقبة الحالة الالتهابية بالتوازي مع مستويات البروتين التفاعلي C.
إن الإنتان بجراثيم الملويات البوابية أو الفيروس المضخم للخلايا قد لا يكون له دور في استمرارية العملية الالتهابية في الأمعاء عند الإنسان.
Abstract EN
Objective: To determine levels of some interleukins with C reactive protein (CRP) and proportion of Helicobacter pylori (HP) and Cytomegalovirus (CMV) infection in Indeterminate Colitis (IC) in Baghdad.
Methods: This cross-sectional study was done in Al-Kindi Teaching Hospital in Baghdad.
Cases group consisted of 60 patients aged between 19 and 84 years who were consulted the hospital between January 2008 and January 2010.
Control group consisted of 30 healthy volunteers who were frequency matched to cases by age and sex.
Serum samples were studied for IL-10, TNFalpha, CRP and H.
pylori IgG antibodies.
Patients were evaluated for CMV by using serology (IgM and IgG antibodies).
Results: Mean level of IL-10 in IC cases was (39.3±15.2 pg/ml) which is significantly higher than control group (p-value=0.024, 95% confidence interval for the difference 1.57, 21.87).
The level of another cytokine (TNF-α) was (22.36±9.04 pg/ml) in indeterminate colitis cases which is significantly lower than the level in control group (33.31±0.796 pg/ml), (p-value=0.000, 95% confidence interval for difference -13.3, -8.6).
Proportions of CRP, H.
pylori IgG and CMV IgM were not significantly different from control group.
Conclusions: These results indicate that IL-10 and TNF-α could be of value in monitoring inflammation along with CRP.
H.
pylori and CMV infection might not have a role in maintaining inflammation in human intestine.
