هدف البحث: تعتبر فطارات الأذن من الحالات السريرية الشائعة التي تتطلب معالجة طويلة الأمد مع ميلها للنكس.
يعتبر تنظيف قناة الأذن الخارجية و خاصةً الجزء الصماخي العميق الأمامي من الأمور الأساسية في العلاج.
يهدف هذا البحث إلى تحديد التظاهرات السريرية و العوامل المؤهبة لحالات فطارات الأذن، و مقارنة الفعالية بين طريقتين من طرق تنظيف الأذن، و تقييم دور طريقة التنظيف بالسوائل في الإزالة الفعالة و الآمنة للتراكمات الفطرية في صماخ الأذن الأمامي العميق.
طرق البحث: تم إجراء دراسة مقارنة مستقبلية شملت 91 مريضا (99 أذن مصابة) في وحدة أمراض الأذن و الأنف و الحنجرة في مستشفى السليمانية التعليمي خلال الفترة بين نيسان 2007 و حتى تشرين الثاني 2008.
تم تقسيم مرضى البحث إلى مجموعتين: خضعت المجموعة A لتنظيف الأذن الرطب )باستخدام السوائل(، بينما خضع مرضى المجموعة B لتنظيف جاف للأذن.
تم عرض و تحليل البيانات باستخدام برنامج Statgraphics الحاسوبي النسخة 15 لعام 2008.
النتائج: شملت التظاهرات الأساسية الملاحظة للحالة ما يلي: ألم في الأذن (94.5 %), حس امتلاء في الأذن (74.72 %), حكة الأذن (58.24 %), نقص سمع (40.65 %), مفرزات من الأذن (20.87 %) و صداع (14.82 %).
شملت العوامل المؤهبة للإصابة كلا من رطوبة الأذن (94.5 %), عادات تنظيف الأذن الذاتية (62.63%), فرط استخدام الأدوية الأذنية الموضعية (36.26 %) و الفطارات الجلدية (19.78 %).
تم من خلال طريقة التنظيف الجاف الوصول لإزالة كاملة للتراكمات في 31 حالة (63.26%) و إزالة جزئية في 18 حالة (36.73 %), أما استخدام طريقة التنظيف الرطب فقد مكنت من الوصول لإزالة كاملة للتراكمات في 38 حالة (76 %) و إزالة جزئية في 12 حالة (25 %).
سجل حدوث اختلاطات بنسبة 37.37 % في مجموعة التنظيف الجاف مقارنة بـ 12.12 % في مجموعة التنظيف الرطب.
الاستنتاجات: لم يلاحظ وجود فروقات هامة إحصائيا بين طريقتي التنظيف المعتمدتين (اختبار كاي مربع، p=0.3411), و لذا يمكن تطبيق أي منهما دون أفضلية.
إلا أن طريقة التنظيف الرطب (مع استخدام السوائل) أقل استهلاكا للوقت و أقل تعقيدا، كما أنها أكثر دقة في تنظيف المنطقة العميقة أمام الطبلة.
Abstract EN
Objective: Otomycosis is not uncommon clinical problem requiring a long term treatment and has a recurrence tendency.
Meticulous cleansing especially of anterior metal recess is the treatment mainstay.
The aim of this work is to determine the clinical presentation and predisposing factors, compare the efficacy of aural toileting procedures and to assess the syringing role as a safe effective procedure for deep meatal recess debris removal.
Methods: A randomized prospective comparativestudy conducted on 91 patients (99 ears) in ENT consultation clinic- Sulaimani Teaching Hospital from April 2007 till November 2008.
Patients were divided into two groups: group A underwent ear syringing and group B underwent dry suction cleansing.
Data entry and analysis carried out by (Statgraphics) software version 15 (2008).
Results: Presenting symptoms were otalgia (94.5%), ear fullness (74.72%), itching (58.24%), hearing impairment (40.65%), otorrhea (20.87%) and headache (14.82%).
Predisposing factors were ear moisture (94.5%), self cleansing habit (62.63%), ototopical agent overuse (36.26%) and dermatophytosis (19.78%).
Dry cleaning showed complete debris removal in 31(63.26%) ears and incomplete removal in 18 (36.73%) ears while syringing showed complete removal in 38 (76%) ears and incomplete removal in 12 (24%) ears.
Dry cleaning adverse effect was 37.37% compared to 12.12% in wet group.
Conclusions: There was insignificant statistical differences (Chi-squared, p=0.3411) between the two regimens and both can be used for treatment.
However, clinically gentle ear syringing is less time consuming, less complicated and more meticulous in deep recess cleaning
