الهدف : تهدف هذه الدراسة إلى إمكانية قياس الأجسام المضادة للسترولينتيد ببتيد في مصل الأطفال المصابين بمرض الرثيان المفصلي، كما استهدفت دراسة أهميته الإكلينيكية.
الطريقة : أجريت هذه الدراسة للسترولينتيد ببتيد بمصل الدم، كذلك اشتملت الدراسة على ثلاثين من البالغين المصابين بالرثيان المفصلي و عشرين من الأطفال الأصحاء كمجموعة مقارنة.
و لقد تم دراسة علاقة الأجسام المضادة للسترولينتيد ببتيد بمواصفات المرض و العلاج الدوائي و إصابة الأشعة.
النتائج : نتج عن هذه الدراسة وجود الأجسام المضادة للسترولينتيد ببتيد بنسبة حوالي 10 % في مرضى الأطفال المصابين بالرثيان المفصلي، و حوالي 67 % في المرضى البالغين، كما كانت نتيجته سلبية في كل الأطفال الأصحاء.
و لقد وجد أن هؤلاء الأطفال المصابين بالرثيان المفصلي كانوا اثنين من الإناث و ذوي دلالة إيجابية لمعامل الروماتويد، و نوع مرضهم متعدد المفاصل.
ثبت أن هناك اختلافا ذو دلالة إحصائية بين المرضى الإيجابيين و السلبيين للأجسام المضادة للسترولينتيد ببتيد و إصابة الأشعة و معامل الروماتويد، بينما لم تختلف إحصائيا مدة المرض و العلاج و وجود الأجسام المضادة للنواة بين مجموعتي المرضى السابقين.
الاستنتاج : استنتج من هذه الدراسة أن الأجسام المضادة للسترولينتيد ببتيد يمكن قياسها في مصل الدم لمرضى الرثيان المفصلي عند الأطفال، و لكن بنسبة اقل تكرارا من البالغين المصابين بالرثيان المفصلي.
و لقد وجد أن الأجسام المضادة للسترولينتيد ببتيد مقصورة على المرضى متعددي المفاصل المصابة و ذوو نتيجة إيجابية لمعامل الروماتويد.
Abstract EN
Objective : to determine if anti-cyclic citrullinated peptide antibodies (Anti-CCP) can be detected in sera of with juvenile rheumatoid arthritis (JRA) patients and to study its clinical significance.
Methods : serum anti-CCP antibodies were measured with ELISA technique in 20 JRA patients.
Thirty adult RA patients & 20 apparently healthy children were also included in the study.
Correlations between anti-CCP, disease characteristics, medication & radiological damage were also determined in JRA patients.
Results: Anti-CCP was positive in 10 % (2 / 20) of JRA patients & in about 67 % (20 / 30) of adult RA patients while it was not detected in healthy children.
The two JRA patients were girls with seropositive (IgM-RF) polyarticular subtype.
There was a statistically significant difference in radiological damage and rheumatoid factor seropositivity between anti-CCP positive and negative JRA patients.
On the other hand, disease duration, antinuclear antibody positivity & medication did not differ statistically between the previous two groups.
Conclusion : Anti-CCP antibodies can be detected in the sera of JRA patients but less frequently present than adults with RA.
Anti-CCP antibodies are exclusively present in the subset of seropositive polyarticular JRA.
