الهدف : دراسة تأثير العلاج بفيتامين ب على مستوى الهوموسيستيين في مرضى الرثيان المفصلي و تحليل العلاقة بين مستوى الهوموسيستيين مع متنوعات الالتهاب و / أو العلاج بالميثوتريكسات.
الطريقة : أجرى البحث على 62 مريض بالرثيان المفصلي و كان مستوى الهوموسيستيين لديهم فوق أو عند مستوى 12 ميكرو مول / لتر و تم تقسيم المرضى عشوائيا لمجموعتين أعطيت المجموعة الأولى فيتامين ب6، ب12 و حمض الفوليك و مجموعة أخرى مادة لها نفس الشكل و لكن بدون فاعلية (بلاسيبو).
و تم علاج المرضى في المجموعتين و تم تقسيمهم في المجموعتين بدون معرفة نوع العلاج بواسطة الطبيب المعالج و الطبيب المسئول عن التقييم خلال فترة 12 شهر (طريقة العمى المزدوج).
النتائج : انخفض مستوى الهوموسيستيين بدلالة مفيدة في مجموعة المرضى يتناولون فيتامين ب و ذلك بالمقارنة بالمجموعة الثانية التي تأخذ العلاج المشابه في الشكل.
و وجد أن هناك ارتباط بين مستوى الهوموسيستيين و مستقبلات الإنترليوكن-2 المذابة عند بداية البحث.
و كذلك ارتباط بين التغير في مستوى الهموسيستيين خلال 12 شهر و العلاج بالميثوتريكسات و كذلك التغير في بروتين سي التفاعلي.
الاستنتاج : يمكن تخفيض مستوى الهوموسيستيين في مرضى الرثيان باستخدام فيتامين ب.
مستوى الهوموسيستيين في مرضى الرثيان المفصلي مرتبط بعلامات الالتهاب و مع العلاج بالميثوتريكسات.
Abstract EN
Objective : to study the effects of treatment with vitamin B on homocysteine (Hcy) levels in rheumatoid arthritis (RA) patients, as well as analyze the relationship between Hcy levels and inflammatory variables, and / or methotrexate (MTX) treatment.
Methodology : Sixty-two RA patients and levels of Hcy ≥ 12 µmol / L were randomized to receive placebo or a combination of vitamins B6, B12 and folic acid.
The patients were treated and evaluated in a double-blind manner over 12 months.
Results : Hcy level decreased significantly in the B-vitamin treated patients as compared with the placebo treated patients.
In a simple regression model, the Hcy level at inclusion was associated with IL-2sR∝.
In a multiple regression model there was an association between the alteration in Hcy level over 0-12 months and MTX treatment, as well as the alteration in CRP, adjusted for B-vitamin treatment.
Conclusion : in RA patients, Hcy level can be reduced with B-vitamins treatment.
Hcy level was related to markers of inflammation and MTX treatment.
