For this clinical study, fifteen patients were selected.

All patients had angel class 1 maxillomaniblar relation.

They had periodontal affected dentition with opposing vertical stoppers that preserve an acceptable vertical dimension of occlusion.

Patients were randomly classified into three equal groups.

Group (A) : patients received conventional immediate mandibular dentures.

Group (B) : patients received immediate mandibular over dentures supported by dome shaped canines.

Group (C) : patients received immediate mandibular over dentures supported by two implants in the canines region.

Cephalometric films were made for patients before extraction of teeth, after the immediate dentures insertion, and after twelve months.

For all groups, there was statistically significant decrease in vertical dimension of occlusion and the sagittal maxilla-mandibular relation increased.

Comparing the three groups, there was a statistically significant difference between group A and groups (B and C) while there was no statistically significant difference between Group B and Group C in all cephalometric measurements.

It was concluded that, the loss of occlusal vertical dimension is due to residual ridge desorption.

It was also coupled with a habitually protruded mandible.

The provision of complete denture patients with dental implant may preserve the alveolar bone desorption and decrease changes in vertical and sagittal maxillomandibular relations.
