هدف البحث : تحديد مستوى دلائل التهاب المفاصل الكيميائية لمرضى العضال العظمى المفصلي الأولى و إيجاد العلاقة بين هذه الدلائل الكيميائية و شدة المرض المحددة إكلينيكيا و بالأشعة.
طريقة البحث : أجرى هذا البحث على 30 مريض بالعضال العظمى المفصلي الأولي و 15 شخص صحيح كمجموعة ضابطة.
و قد تم فحص جميع الأشخاص إكلينيكيا و عملت لهم في المصل و كذلك إنزيم التشو انهبتور-1 في البلازما و البريدونولون في البول.
النتائج : و قد أظهرت النتائج أن مستوى الميتالوبروتينيز-3-و التشو انهبتور-1 في الدم و البيريدينولين في البول أعلى في مرضى العضال العظمي المفصلي مقارنة بالمجموعة الضابطة و كان الفرق ذو دلالة إحصائية عالية.
أما بالنسبة للتغيرات المفصلية المشخصة بالأشعة وجدت علاقة طردية بين دلائل الالتهاب الكيميائية و ضيق المسافة المفصلية.
الاستنتاج : من الممكن استخدام بعض دلائل الالتهاب المفصلي الكيميائية لتشخيص و تحديد شدة المرض في مرضى العضال العظمي الأولي و باستخدام مثبطات الميتالوبروتينيز-3 يمكن تقليل و تأجيل تدهور و تآكل المفاصل.
Abstract EN
Objectives : to find out whether serum level of MMP-3, plasma level of TIMP-1 and urinary pyridinoline are specifically increased in generalized osteoarthritis (OA) or not.
Also whether there is a relationship between those markers and the disease severity as detected clinically and radio logically.
Methods : Thirty females suffering of generalized OA and 15 apparently healthy matched females as controls were studied.
Serum MMP-3, plasma TIMP-1 and urinary pyridinoline were measured.
The knee and hand joints were graded clinically (Stein rocker) and radio logically (Kellgren and Lawrence).
Results : serum level of MMP-3, plasma level of TIM-1 and urinary pyridinoline were significantly higher in generalized OA patients than normal controls.
The joint space width decreased with increasing Kellgren–Lawrence grade.
All biochemical markers had negative correlations with the joint space width, but only urinary pyridinoline had a significant correlation.
All biochemical markers had positive correlations with Stein rocker grading.
Conclusion : some biochemical markers of OA may be of diagnostic value and a predictor for the severity of the disease in progressive generalized OA.
The inhibition of their production and activity may decrease or delay the joint damage.
