الطريقة : أجريت هذه الدراسة على 24 مريضا (9 من الرجال و 15 من الإناث) مصابين بسرطان الثدي و الرئة (غير صغير الخلية) و المبيض من المترددين على وحدة الأورام بمستشفيات جامعة الزقازيق-ما بين مارس 2003 و ديسمبر 2004.
و قد تم تقسيم المرضى إلى مجموعتين : مجموعة (1) تتلقى فيتامين هـ مع العلاج الكيماوي مكونة من 12 مريض و مجموعة (2) بنفس العدد لا تتلقى فيتامين هـ مع العلاج الكيماوي.
و قد اشتمل تقييم المرضى قبل بدء العلاج و بعد 6 دروات من العلاج الكيماوي على : التاريخ المرضى و الفحص الإكلينيكي و تحليل كامل للدم وظائف الكبد و الكلى وتحليل دلالات الأورام و عمل رسم للقلب و الكشف بالأشعة العادية على الصدر و الكشف بالأشعة التليفزيونية على البطن و الحوض و القلب و كثير من الحالات بالأشعة المقطعية على البطن و الحوض و عمل مسح ذري للحالات التي تحتاج إلى ذلك.
و قد تم الكشف الإكلينيكي العصبي الكامل على المرضى و تم عمل قياس سرعة التوصيل العصبي بقسم الروماتيزم و التأهيل على العصب الزندي و الشظوي و الربلي.
النتائج : و قد أثبتت النتائج أن التهاب الأعصاب الطرفية قد حدث بنسبة أقل في مرضى الأورام الذين تناولوا فيتامين هـ مع العلاج الكيماوي عن المرضى الذين أعطى لهم العلاج الكيماوي فقط، و كان الفارق ذات دلالة إحصائية (80 % من مرضى المجموعة الأولى مقابل 27.3 % من مرضى المجموعة الثانية).
الاستنتاج : أن استخدام فيتامين هـ مع العلاج الكيماوي في مرضى الأورام يلعب دورا هاما في الوقاية من التهاب الأعصاب الطرفية الناتج من استعمال العلاج الكيماوي، و نوصي بعمل دراسة أكبر لتأكيد هذه النتائج.
Abstract EN
Objective : to evaluate the neuroprotective effect of Vitamin E supplementation in cancer patients receiving chemotherapy.
Methodology : this study was carried out on 24 patients with breast, ovarian and lung cancer.
They were divided into two groups: Group (I) including 12 cancer patients on chemotherapy with 600 mg Vitamin E supplementation and Group (II) including 12 cancer patients on chemotherapy without vitamin E supplementation.
All patients were subjected to full medical and neurological examination, nerve conduction velocity study (NCV) of surely, ulnar and personal nerves.
Measurement of neurotoxicity score was done and was classified as no toxicity, mild, moderate and severe toxicity.
Results : only 21 patients completed this study and we found a significant (p < 0.05) reduction of ulnar and surely sensory nerve action potentials (SNAP) in group (II) after treatment with chemotherapy in comparison to that before treatment.
On the other hand, no significant slowing of NCV or reduction of M response was detected.
We also found a significant reduction (p < 0.05) of SNAP of ulnar and sural nerves in group (II) in comparison to patients in group (I) after taking chemotherapy.
Conclusion : vitamin E supplementation may have a role in amelioration of peripheral neuropathy induced by chemotherapy.
