مقدمة : أهم الأسباب المرضية المرتبطة بأنيميا البحر المتوسط هي هشاشة العظام و من المعروف أن أسباب هشاشة العظام في هؤلاء المرضى متعددة مثل خلل في أيض فيتامين د أو التسمم بالحديد و أيضا نشوء المرض غير واضح و كيفية عمل (cytokines) سيتوكينز على العظام.
حديثا تم اكتشاف نظام (RANK / Osteoprotegrin) و الذي أثبت أن له دور فعال في حدوث أنيميا البحر المتوسط.
الطريقة : لقد تم إجراء الدراسة على 25 طفلا يعانون من أنيميا البحر المتوسط و عشرة أطفال أصحاء و لقد تم فحص الجميع إكلينيكيا و أخذ التاريخ المرضي لهم.
و لقد تم قياس مستوى فيتامين د و مؤشرات الهدم للعظام بالإضافة إلى مستوى الأستيوبروتيجرين و مقارنتهم بمقياس كثافة العظام.
النتائج : و لقد أظهرت الدراسة أن الأطفال المصابين بأنيميا البحر المتوسط يعانون من فارق ذو دلالة إحصائية في مستوى 25 هيدروكسي فيتامين د مقارنة بالمجموعة الضابطة، أيضا وجد أن مستوى مؤشرات الهدم (DpD, Dyp) كان بها زيادة ذات دلالة إحصائية عالية.
و عند قياس مستوى الأستيوبروتيجرين كان أقل من المستوى الطبيعي.
كما وجد علاقة تناسب عكسي بين مستوى (DpD, Dyp) و مستوى الأستيوبروتيجرين وفيتامين د.
و من الملاحظ أن مرضى أنيميا البحر المتوسط يعانون نقص في مستوى كثافة العظام و أيضا بقياس مستوى البلوغ كان أقل مقارنة بالمجموعة الضابطة.
الخلاصة : أن هشاشة العظام الناتجة في أنيميا البحر المتوسط مشكلة كبيرة ذات تحديات شديدة.
و من الملاحظ أن الأستيوبروتيجرين له دور فعال و يحتاج إلى المزيد من الاختبارات.
Abstract EN
Hypothesis: osteoporosis represents an important cause of morbidity in thalassemia major (TM) patients.
The cause is multifactorial ; including defective vitamin D metabolism and iron intoxication.
The pathogenesis is not completely clarified, especially the mechanism by which cytokines affect skeletal bones.
Osteoprotegrin / RANK system has been recently introduced in the pathogenesis of post-menopausal osteoporosis and other bone diseases.
Objective : to investigate the relationship between vitamin D, as a bone resumption marker and the new bone cytokine (osteoprotegrin) and their implication on bone mineral density (BMD).
Methodology : Twenty-five TM patients and 10 healthy controls were included in the study.
They were subjected to full history and clinical examination.
Body mass index was assessed.
Serum 25 (OH) 2 Vitamin D was estimated.
Bone resumption markers (Deoxypyridinoline and pyridinoline cross links) were assessed.
Osteoprotegrin serum level was measured and BMD was assessed with DEXA.
Results : serum 25 (OH)2 Vitamin D was significantly reduced in patients than controls.
Bone resumption markers (DpD, pyD) showed a highly statistical significant increase than controls.
Also, serum osteoprotegrin was reduced than controls.
Moreover, there was a negative correlation between the levels of PyD, DpD and osteoprotegrin.
Again, TM patients had lower bone mineral density and Tanner staging than controls.
Conclusion : TM osteoporosis is a major problem that is a real challenging issue.
Osteoprotegrin / RANK system has got a direct role which needs further investigation.
