هدف البحث : يهدف هذا البحث إلى المقارنة بين نتائج عملية استئصال الزائدة بالطريق المفتوح مع الاستئصال بتنظير البطن بغية تحديد الفوائد العلاجية لتقنية التنظير في تدبير حالات التهاب الزائدة الحاد المنفجرة.
طرق البحث : شملت الدراسة 60 مريضا عولجوا في المستشفى الحكومي لعاصمة جنوب إفريقيا خلال العام 2009 بتشخيص التهاب زائدة حاد مترافق مع انفجار الزائدة، تم علاج 31 حالة من خلال تنظير البطن، بينما احتاجت 29 حالة إلى المعالجة باستئصال الزائدة بفتح البطن.
النتائج : لوحظ أن مدة الإجراء بالمنظار كانت أطول و بشكل هام من الوقت الذي استغرقته عمليات فتح البطن (120 دقيقة مقابل 60 دقيقة على الترتيب)، إلا أن نسبة الإنتانات داخل البطن بعد العملية بجراحة المنظار كانت أقل بالمقارنة مع العلاج عن طريق فتح البطن (4.6 % مقابل 7.13 % على الترتيب).
لم تظهر المجموعتان وجود اختلافات فيما بينهما من حيث مدة المكوث في المشفى.
لوحظ أن نتائج الجراحة بفتح البطن تتأثر بالعمر، مدة الأعراض السريرية عند المريض و التظاهرات الملاحظة للحالة و منها ارتفاع تعداد كريات الدم البيضاء.
الاستنتاجات : يمكن من خلال هذا البحث القول بإمكانية تدبير حالة التهاب الزائدة الحاد المنفجرة داخل جوف البطن من خلال المنظار الجراحي، و ذلك كون هذه التقنية آمنة و سهلة التطبيق، و أقل تأثرا بالموجودات السريرية للحالة قبل العملية علاوة عن كونها أقل تسببا بالاختلاطات، و لهذا فهي تمثل التقنية المفضلة لتدبير هذه الحالات.
Abstract EN
Objective : The aim of this study is to compare the different outcomes of laparoscopic with open appendectomy and ascertain their therapeutic benefits, if any, in the overall management of perforated acute appendicitis.
Methods : Data was collected from 60 cases admitted to a governmental hospital in South Africa over a one year interval (2009) with an intraoperative diagnosis of perforated acute appendicitis, 31 of them were managed laparoscopically (LA) while the other 29 cases were approached by an open appendectomy (OA).
Results : The procedure duration was significantly higher in the laparoscopic group (120 minutes for LA as opposed to 60 minutes for OA) ; the rate of intraabdominal sepsis was found to be lower in the LA group (6.4 % for LA compared to 13.7 % for OA).
Both groups showed no significant statistical difference in the hospital length of stay.
Also it was observed that the age, symptoms’ duration, clinical presentation and the white blood cell count (WBC) were influencing factors to the outcome measures of the OA group.
Conclusions : We conclude that perforated acute appendicitis can be managed successfully laparoscopically.
It is safe, feasible ; less influenced by preoperative findings and shows a trend towards less postoperative complications and can be considered as the procedure of choice for perforated acute appendicitis.
