يهدف هذا البحث إلى تحديد معدل تحويل عملية استئصال المرارة من المنظار إلى الجراحة التقليدية و كذا تحديد أسباب التحويل.
لقد تمت دراسة و تحليل المعلومات الخاصة بالمرضى الذين يعانون من حصوات في المرارة و الذين تم تحديدهم لإزالة المرارة عندهم بالمنظار في قسم الجراحة بمستشفى الكويت الجامعي في صنعاء و ذلك في الفترة من يناير 2002 م و حتى ديسمبر 2006 م مركزين في ذلك على العلاقة بين معدل التحويل و الأسباب التي تؤثر في عملية التحويل إلى الجراحة التقليدية.
الدراسة اشتملت على 709 مريضا موصوفين لإزالة المرارة عندهم بالمنظار منهم 648 إناثا و 61 ذكورا عند معدل عمر 38 عاما (نطاق 14-86 عاما).
و استثنينا من دراستنا تلك الحالات التي كانت موصوفة إلى الجراحة التقليدية منذ البداية.
بلغ عدد عمليات استئصال المرارة بالمنظار بنجاح 650 عملية بمعدل نجاح بلغ 91.7 %، بينما تم تحويل 59 عملية استئصال المرارة من المنظار إلى الجراحة التقليدية بمعدل إخفاق (تحويل) 8.3 % من مجموع العمليات التي أحيلت لإجرائها بالمنظار.
و تم تحديد عدة أسباب تحويل إزالة المرارة بالمنظار إلى الجراحة التقليدية، و من أهم تلك الأسباب الالتصاقات الكثيفة بين الأنسجة عند 20 مريضا بمعدل 33.9 %.
أظهرت دراستنا الوصفية إن معدل تحويل عملية إزالة المرارة من المنظار إلى الجراحة التقليدية عند جراحي المناظير اليمنيين ما زال مرتفعا، عند مستوى البدايات الأولى لإزالة المرارة بالمنظار و من أهم أسباب التحويل الالتصاقات الكثيفة بين الأنسجة.
كما إن غياب نظام صارم و نقص الخبرة في جراحة المناظير يزيد من معدل التحويل.
Abstract EN
The objective of this study is to audit the conversion rate and to identify the reasons necessitating laparoscopists in our department so as to convert laparoscopic cholecystectomy to open procedure.
The data of patients who were scheduled for laparoscopic cholecystectomy (n = 709) between 2002 and 2006 were retrospectively analyzed in relation to conversion rate and reasons for conversion.
The study included 648 women and 61 men with the mean age of 38 years.
(range 14 to 86 years).
Cholecystectomy that were not primarily scheduled to laparoscopic cholecystectomy from the beginning were excluded.
Laparoscopic cholecystectomy was successfully performed in 650 patients with completion rate of 91.7%.
Conversion to open cholecystectomy was performed in 59 patients with a conversion rate of 8.3%.
Various reasons have been identified.
Extensive adhesions were the most common reason for conversion (n = 20, 33.9%).
In conclusion, our descriptive study has shown that laparoscopists in our department still have conversion rate as high as that being reported in early years of laparoscopic cholecystectomy.
The most common reason for the conversion to an open procedure was dense and extensive adhesions.
The absence of strict guidelines and lack of an extensive experience in laparoscopic surgery increased the rate of conversion.
