صممت هذه الدراسة لمعرفة تأثير استعمال نخاع العظم في التئام وتر القابضة الإصبعية المحدثة تجريبيا في الأغنام.
استعمل للدراسة الحالية 8 رأس غنم بالغ تراوحت أعمارها 9 أشهر إلى 1 سنة، قسمت عشوائيا بالتساوي إلى مجموعتين حيث عدت المجموعة الأولى كمجموعة سيطرة و الثانية مجموعة معاملة بالرقعة النمطية.
استحدث قطع جراحي مستعرض لوتر القابضة الإصبعية السطحية عند الثلث الوسطي للعظم المشطي الثالث في القائمة الخلفية تحت تأثير المهدى و المخدر الموضعي و من ثم خيط الوتر المقطوع بخياطة Bunnell باستعمال خيوط البولي بروبيولن قياس 2.0 في مجموعة السيطرة، بينا في مجموعة الرقعة النمطية تم وضع 2-3 مل من مادة نخاع العظم النمطي و حقنت في منطقة التفمم بين نهايتي الوتر و من ثم طبقت نفس الاجرأت التي تمت في مجموعة السيطرة و غلق الجرح بالطرق التقليدية.
أظهر الفحص العياني وجود التصاقات بين الوتر و التراكيب المجاورة و التي كانت أشد في مجموعة السيطرة، و أقل منها في مجموعة الرقعة النمطية.
أظهر الفحص النسجي المرضي للخزعات التي تم الحصول عليها بعد 15، 30، و 60 يوم أظهرت وجود ألياف غراوية غير ناضجة تميزت بانتظامها و انتظام مكوناتها الخلوية التي ملأت منطقة القطع للوتر مع كثرة الالتصاقات في المراحل المتقدمة من مجموعة السيطرة، بينما في مجموعة المعالجة بالرقعة النمطية، الخلايا الالتهابية و انتظام النسيج الليفي مع الألياف الغراوية ظهرت أكثر مما موجود في مجموعة السيطرة في نفس مع قلة الالتصاقات.
Abstract EN
The present study was to compare the effectiveness of bone marrow transplantation on the healing of experimentally transected superficial digital flexor tendon in sheep.
Eight adult sheep, aged between 9 months to one year were used.
The animals were divided into two equal groups (control and allograft).
A transverse cutting has been made in the superficial digital flexor tendon of the middle third of the metatarsal bone under the effect of sedation and local anesthesia.
The tendons were sutured with Bunnell suture by using polypropylene thread (2.0 usp) in the control group.
While in allograft groups 2-3ml allograft bone marrow was injected at the site of suture tendon which was induced by similar procedure as in control group.
The incision was closed as a routine manner.
Gross examination showed adhesions between the tendon and adjacent structures which were severe in the control group, less than that in allograft.
Histopathologically observations of biopsies harvested after (15, 30 and 60 days) showed dense immature collagen fibers which was characterized by regular direction, uniform of their cellular elements filled the gap space of tendon injury with more adhesion in late stages of the control group.
In allograft treated group inflammatory cells and arrangement fibrous connective tissue with collagen fibers appear more than in the control group in the same period with less adhesion.
