الھدف من الدراسة ھو التعرف على استخدام المضادات الحیویة قبل إجراء العملیات الجراحیة و دوره في تقلیل نسبة ألالتهابات و المضاعفات الناتجة بعد العملیات الجراحیة.
تم دراسة الملفات الطبیة لدى 103 من المرضى الأطفال الذي تم إدخالھم إلى قسم جراحة الأطفال بمستشفى الجمھوریة التعلیمي في الفترة من یونیو-أكتوبر 2004 و تتراوح أعمارھم من یوم إلى 15 سنة ذكور و إناث، و كان التشخیص الأولي یشمل حالات مختلفة، و تم رصد الحالات التي تلقت المضادات الحیویة قبل إجراء العملیات الجراحیة في فترة 24 إلى 48 ساعة و متابعة ظھور الالتهابات و المضاعفات فیما بعد.
45.6 % من المرضى تلقوا المضادات الحيوية قبل إجراء العمليات الجراحية، و كان 74.3 % منهم يوجد تحت تشخيص العمليات المتقيحة، و كان 90 % منهم يعانون من الدمامل بالرغم من استخدامهم المضادات الحيوية قبل إجراء العمليات الجراحية لكن ثلاث حالات لديها التهابات و مضاعفات ناتجة عن العمليات الجراحية.
77.3 % هي نسبة الفئة العمرية الأكثر استخداما للمضادات الحيوية و التي تقع بين 10 إلى 15 سنة.
أما بالنسبة للفترة الزمنیة لوجود المریض داخل المستشفى و علاقتھا بحدوث نسبة ألالتهابات و المضاعفات الناتجة بعد العملیات فالنتائج لم تظھر أي دور أساسي لھا.
إن استخدام المضادات الحیویة للمریض قبل إجراء العملیات الجراحیة یؤدي إلى التقلیل من نسبة حدوث ألالتهابات و المضاعفات الناتجة بعد العملیات و لكن ذلك لا یمنع من ظھورھا و ذلك للأسباب التالیة : 1.
استخدام طرق التعقیم الحدیث للأدوات و الأجھزة الطبیة الجراحیة في غرف العملیات و كذلك الأقسام الجراحیة.
2.
الممیزات الخاصة بكل مریض من عوامل تغذیة و مناعة و عوامل وراثیة و غیرھا.
3.
التحضیر الجید للمریض قبل إجراء العملیات أثناء العملیات و العنایة المركزة للمریض بعد العملیات.
Abstract EN
With the objective to identify different aspects of the use of preoperative antibiotics and its relationship with postoperative complications, one hundred and three medical records from children who were admitted in the Children Surgical Ward in Al-Gamhouria Taching Hospital from January to October 2004 were randomly selected and studied.
Age ranged from 1 day to 15 years.
In 45.6 % patients, antibiotics were used preoperatively; also in 74.3 % of the septic operations, antibiotics were used preoperatively.
Ninety percent of the cases diagnosed were having an abscess and 58.8% were diagnosed as appendicitis and received antibiotics preoperatively.
Postoperative infections appeared in 11.1 % and 10 % of these cases respectively.
In 77.3 % patients in the group of age 10-15 years old, antibiotics were used preoperatively.
There were no differences in patients who used antibiotics preoperatively and in the hospitalization period.
It is concluded that antimicrobial drugs used preoperatively can reduce the risk of postoperative infections, but do not prevent it because primary prophylactic measures against postoperative infections include a group of factors like antiseptic technique, individual patient characteristics, and patient’s preparation before surgery, and postoperative patient care.
