Aim of the work : This prospective study was carried out at the National Cancer Institute, Cairo University.

The aim of this study is to evaluate the use of the ileocecal segment as a viginal substitute in young female patients undergoing vaginal resection for malignant pelvic tumors.

Patients and Methods : The study included eleven patients with different pelvic malignancies undergoing vaginal resection as a part of surgical treatment.

The ileocecal segment, based on the ileo-colic artery, was used for vaginal reconstruction in all cases.

Five cases had cervical carcinoma that underwent extended Wertheims operation.

Three cases had urinary bladder cancer that underwent anterior pelvic excentration.

Two cases had endometrial carcinoma, and one case had ovarian tumor.

In 2 cases the terminal ileum was used, in addition, for ureteric replacement in one case, and as a bladder substitute in the other case.

Their ages ranged from 39 to 47 years, with a mean age of 42 years.

Primary reconstruction was done in 9 cases, while secondary reconstruction was performed in 2 cases.

Results : An average of one hour was consumed in addition to the time of the original operation.

Mucous discharge, as an early post operative complication was recognized only in 2 cases (18 %).

Late complications, in the form of inspissated mucous secretion, were encountered only in 3 cases (27 %).

Sexual function was satisfactory in most of the cases (10 cases) (90.9 %).

Conclusions : The ileo-coecal segment, although not the ideal, is an excellent substitute for vaginal reconstruction, with minimal post operative complications and satisfactory sexual function.
