سوء التغذية يصيب 70–40 % من المصابين بالفشل الكلوي المتقدم.
أسباب سوء التغذية في المرضى الذين يعانون من أمراض الكلى المزمنة تشمل انخفاض استهلاك الغذاء، انخفاض امتصاص الأمعاء و الهضم و الحماض الاستقلابي.
و يشتبه في وجود سوء التغذية من القياسات البشرية أو من نقص ألبومين الدم أو من انخفاض إنتاج الكرياتينين.
القياسات البشرية توفر طريقة سريعة و متكاثرة، و سهلة، و غير اجتياحية لتقييم الدهون في الجسم و كتلة العضلات.
الهدف : الهدف من الدراسة هو تقييم الحالة التغذوية للمرضى الذين يعانون من الفشل الكلوي المتقدم الذين هم تحت الديلزة الدموية المنتظمة باستخدام القياسات البشرية.
المنهجية : دراسة استشرافية، مقطعية مستعرضة أجريت في مستشفى الجزيرة لأمراض الكلى و الجراحة في يونيو 2010.
تم ضم 202 مريضا في هذه الدراسة.
و تم قياس مؤشر كتلة الجسم، سمك ثنية الجلد في العضلة ثلاثية الرؤوس، سمك ثنية الجلد تحت الكتف و محيط منتصف الذراع.
النتائج : في 79-70 % من المرضى كان مؤشر كتلة الجسم طبيعيا أو مرتفعا بينما القياسات البشرية الأخرى كانت أقل من القيم القياسية في 83-66 % من المرضى.
يمكن أن تعزى هذه النتيجة إلى احتباس السوائل.
الخلاصة : يمكن أن تكون القياسات البشرية طريقة مفيدة للكشف المبكر عن سوء التغذية و متابعة الحالة التغذوية لمريض الفشل الكلوي المتقدم خاصة في البلدان النامية.
Abstract EN
Background: Malnutrition affecting 40 to 70% of patients with end-stage renal disease.
Cause of malnutrition in patients with chronic renal disease include of a lower food intake, decreased intestinal absorption and digestion, and metabolic acidosis.
The presence of malnutrition is usually suspected from anthropometry or the presence of hypoalbuminemia or decreased creatinine production.
Anthropometric measurements provide a rapid, noninvasive, easy, and reproducible method for evaluating body fat and muscle mass. Objective : the objective of the study is to assess the nutritional status of patients with ESRD on regular hemodialysis using anthropometric measurements. Methodology : Prospective, cross-sectional, study was conducted in Gezira Hospital for Renal Diseases and Surgery on June 2010.
202 patients were joined in this study.
Body mass index (BMI), triceps skinfold thickness, subscapular skinfold thickness and mid-arm circumference (MAC) were measured. Results : 70.79 % of the patients were having normal or high BMI while other anthropometric measurements were below the standard values in 66.83 % of the patients.
This result could be attributed to fluid retention. Conclusion : anthropometric measurements could be useful way for early detection malnutrition and follow up of nutritional status for patients on end-stage renal disease particularly in developing country.
