هذه دراسة عشوائية مقارنة تنبؤيه أجريت في مستشفى كوستي التعليمي في الفترة ما بين 2006-2008 م.
الأهداف : تحديد نتائج التصوير الإشعاعي و ذلك لتقييم مدى تقدم نتائج التصوير الإشعاعي لدى مريض الدرن الرئوي الذي يعالج يوميا إزاء نظيره الذي يتلقى علاجا متقطعا بكميات قليلة.
طرق و وسائل البحث : تم تسجيل ما مجموعه 275 مسحة موجبة لمرض الدرن و من ثم تقسيمهم عشوائيا إلى مجموعتين : مجموعة تتلقى علاجا متقطعا (المجموعة أ) و مجموعة تتلقى علاجا يوميا (المجموعة ب).تم التصوير بالأشعة السينية لدى بداية و نهاية العلاج لكل مريض.
النتائج : وجد أن 31 % من صور الأشعة السينية طبيعية فيما يتعلق بتطور الآفة الأولية وجد أن نسبة عالية مقدرة جدا أظهرت صور أشعة سينية طبيعية بنسبة 58 % في المجموعة (أ) و نسبة 53 % في المجموعة (ب).
42 % من المجموعة (أ) و 4 % من المجموعة (ب) بقيت كما هي تعاني من الآفة الأولية.
نتائج الآفة المتقدمة أظهرت أن العلاج المتقطع حسن 16.6 % من صور الأشعة السينية إلى الحالة الطبيعية، 8.3 % إلى آفة متوسطة الخطورة.
3.33 % إلى آفة متقدمة و لكن بصورة متوسطة بينما في 25 %من الحالات لم يتغير شيء.
بينما عند استخدم العلاج اليومي وجد أن 21 % تحسنوا إلى الصورة الطبيعية أو إلى الآفة الأولية، 5.2 % إلى آفة متوسطة، 27 % إلى آفة متوسطة و لكن بصورة متوسطة.
مما سبق يتضح أن التحليل الإحصائي أظهر اختلافا واضحا بين المجموعتين.
الخاتمة : التحول من آفة أولية إلى صورة أشعة طبيعية أعلى في حالة أخذ العلاج المتقطع لكن التحول من الآفة المتقدمة إلى صورة الأشعة الطبيعية أعلى عند أخذ العلاج اليومي.
Abstract EN
This is a prospective comparative randomized study carried out at Kosti teaching hospital 2006-2008. Objectives : to determine the radiological findings and to evaluate the radiological progress in patient with pulmonary Tuberculosis treated with daily versus intermittent short course regimens. Methodology : A total of 275 smear positive new cases of Tuberculosis were enrolled and randomized in to two groups, intermittent group (A) and daily regimen group (B) .X-Ray film was done initially and at the end of treatment for each patient. Results: It was found that 31 % of the X-Ray films were normal.
Concerning the progress of minimal lesion significantly high percentage improved to normal X-Ray 58% in group A and 53 % in group B, 42 % in group A and 40 % in group B stayed as minimal lesion.
The results of far advanced lesion revealed that intermittent regimen improved 16.6 % of the films to normal and minimal lesion, 8.3 % to moderate lesion, 33.3 % moderately advanced lesion and 25 % remained unchanged, while in the daily regimen, 21 % improved to normal and minimal lesion, 5.2 % to moderate lesion, 27 % to moderately advanced lesion and 27.6 %.
Statistical analysis revealed a significant difference between the two groups. Conclusion : conversion of minimal lesions to normal X-Ray was significantly higher in the intermittent group and the conversion of far advanced lesions to normal was significantly higher in the daily regimen.
