المقدمة : إعطاء الأقراص الحديدية إلى النساء الحوامل في عيادات متابعة الحماة تقليد معمول به في السودان.
الشحن الزائد للحديد و بالتالي الجهد التأكسدي هو خطر محتمل.
الأهداف : هدفنا في هذه الدراسة لمعرفة العلاقة بين مستوى الحديد في الحوامل و نتاج الحمل.
المرضى و الطرق : أجريت هذه الدراسة في ولاية الخرطوم-السودان في الفترة بين ديسمبر 2007-فبراير 2009.
أخذت العينات الوريدية من 123 من الحوامل عند الولادة.
نتائج الحمل غير المرغوبة مثل سابقة التشنج الحملي، انخفاض وزن المواليد، العملية القيصرية و ناقصي الحمل تم تدوينها.
مستوى الحديد و مقاييس الدم أيضا دونت.
النتائج : تم تقسيم الأمهات إلى مجموعات حسب مستوى الحديد في المصل إلى مستوى حديد منخفض ( > LSI 14 = 50mg / dl ,n) مستوى حديد طبيعي (98 =NSI : 50-170 mg / dl, n) و مستوى حديد زائد (:IOL 11= 170mg / dl, n<).
معدل حدوث سابقة التشنج الحملي كان أعلى في مجموعة مستوى الحديد الزائد (% 72.7) تلتها مجموعة مستوى الحديد المنخفض (35.7 %) و كانت أقل معدل حدوث في مجموعة مستوى الحديد الطبيعي (19.4 %).
متوسط أوزان المواليد تم مقارنتها في مجموعتي الحديد الزائد و المنخفض و كانت الأوزان اقل من تلك الأوزان في مجموعة مستوى الحديد الطبيعي.
الخلاصة : إعطاء أقراص الحديد و حوامل يجب أن يرشد حسب الحوجة و الفائدة بدون آثار غير مرغوب فيها.
Abstract EN
Dispensing iron tablets to pregnant women at antenatal clinics is a common practice in Sudan.
Iron overload and, consequently, oxidative stress is a possible risk.
Objective: In this study, we examined the iron status in pregnant women in correlation to pregnancy outcome.
Subjects and methods : The study was conducted in Khartoum state, Sudan in the period December 2007-February 2009.
Venous blood samples were obtained from 123 women at delivery.
Undesirable pregnancy outcomes as preeclampsia, low birth weight, caesarean sections and preterm delivery, if any, were recorded.
Serum iron and hematological parameters were determined.
Results : Mothers were grouped, according to their serum iron levels, as low serum iron (LSI: < 50μg / dl, n = 14), normal serum iron (NSI : 50 - 170 7g / dl, n = 98) and iron overload (IOL: >170 7g/dl, N = 11) groups.
The incidence of preeclampsia was highest among the IOL group (72.7 %), followed by the LSI group (35.7 %) and lowest among the NSI (19.4 %) group, p =.
The mean babies’ birth weights were comparable among the IOL and the LSI groups but both were significantly lower than that among the NSI group.
Conclusion : Iron supplementation to pregnant women must be rationalized so that women will benefit without developing undesirable effects.
