الأهداف : يعمل البروتين الرابط للانترليوكين – 18 (IL-18BP) كمضاد طبيعي للالتهابات و مثبط مناعي جزئي بمعادلته لتأثيرات انترليوكين – 18 اثناء الاتهاب.
هدفت هذه الدراسة لمعرفة الدور الذي يلعبه البروتين الرابط الانتر ليوكين – 18 في تنظيم الاستجابة المناعية المرتبطة بأمارضية التهاب المفاصل الروماتيزمي.
المواد و الطرق : تمت دراسة 65 مريض بالتهاب المفاصل الروماتيزمي, 22 مريض بالتهاب المفاصل المتعظمى و تم مقارنتهم ب 40 متطوع صحيح كمجموعة مقارنة من نفس العمر و الجنس.
تم الحصول على النماذج الزلالية عن طريق تنظير المفصل و البزل.
عينات (SFMC) و (PBMC) تم تحضيرها بواسطة طريقة الفصل فيكول هايباك.
تم استخدام التحليل القياسي لتوضيح الجينات المرتبطة بالمناعة في عينات (PBMC) للأصحاء بعد معالجتها بواسطة البروتين الرابط للانترليوكين – 18 (IL-18 BP) المعد جينيا.
تم قياس مستويات mRNA في محركات الخلايا التالية المساعدة 1 و 2 بواسطة (PCR).
و تم تحديد مستويات بورتين الانترفيرون قاماً, انترليوكين 4 بواسطة اليزا (ELISA).
النتائج : التحليل بواسطة (Super array) للجينات المتعلقة بالمناعة في (PBMC) للأصحاء و التي تمت معالجتها بالبروتين الرابط للانترليوكين – 18 أوضحت انخفاض في الإيضاح الجيني للقاما انتر فيرون و جزيئاته STAT1, STAT2.
أوضحت هذه الدراسة أن للبروتين الرابط بالانترلوكين – 18 خاصية إضافية مضادة للالتهاب من خلال عملها بالتنظيم السلبي لإيضاح القاما انترفيرون و الانترليوكين – 12 و في نفس الوقت لها تنظيم إيجابي لعمل الانترليوكين 2 و 4.
الخلاصة : أوضحت هذه النتائج أهمية البروتين الرابط للانترليوكين – 18 كجزء مناعي منظم واعد في علاج التهاب المفاصل الروماتيزم.
Abstract EN
Objectives : Interleukin-18 binding protein (IL-18BP) is functioning as a natural anti-inflammatory and immunosuppressive molecule by neutralizing the effects of IL-18 during inflammation.
This study aimed to identify the role of IL-18BPa in the regulation of immune responses associated with the pathogenesis of RA.
Materials and Methods : 65 RA patients, 22 OA patients, and 40 sex and age matched healthy donors were enrolled in this study.
Synovial specimens were obtained through synovectomy or arthroscopic procedures.
SFMC and PBMC were prepared by using Ficoll-Hypaque separation procedure.
Superarray analysis was used to measure the expression profile of immune-related genes in normal PBMC treated with recombinant human IL-18BPa.The mRNA levels of Th1 and Th2 cytokines were measured by Real-time PCR, and the protein levels of IFN-y, IL-4 were detected by ELISA.
Results : SuperArray analysis of immune related gene expression profile in normal PBMC treated with IL-18BPa indicated decreases in the gene expression of IFN- y and its regulatory molecules STAT-1 and STAT-2.
This study pointed out that IL-18BPa has additional anti-inflammatory property through downregulating the expression of IFN- y and IL-12, at the same time, upregulating the expression of IL-4 and IL-10.
Both IFN- y and IL-12 could upregulated the mRNA and protein levels of IL-18BPa in both the normal and RA subjects.
Conclusion : Our results demonstrated the importance of IL-18BPa as an immune regulatory molecule and as a promising therapy for treating RA.
