أصبح الاختبار السريري الموضوعي (الأوسكي) أحد الوسائل الواسعة و المقبولة لتقييم كثير من المهارات السريرية لطلاب الطب و الأطباء.
الأهداف : تسهيل الكيفية و تفعيل قبول الأوسكي كوسيلة تقييم معول عليها.
مشاركة المؤسسات الصحية و الطبية و تبادل الخبرة و الاستفادة من تجربة قسم الجراحة بكلية الطب جامعة أم القرى.
المساهمة في تقليل الأخطاء و توفير الجهد و دفع و تشجيع الراغبين في اعتماد (الأوسكي) كوسلية تقييم.
الطريقة : اعتمد قسم الجراحة بكلية الطب جامعة أم القرى الاختبار السريري الموضوعي كأحد دعامات التقييم السريري من أول دفعة للكلية في العام 2000 (1420هـ) حتى وصلت عدد دورات الأوسكي إلى أكثر من عشرين الآن كما قد أمكننا تسيير أربعة اختبارات في وقت واحد طلبة و طالبات.
إن مسئولية التحضير و وضع الأهداف و ترتيب و تسيير الأوسكي و تطويره مسؤلية القسم بقيادة منسق المحطات حيث الإعداد للأسطي يكون مستمراً قبل و أثناء و بعد الاختبار بمسؤوليات و أهداف محددة.
و في كل مرة تزداد التجربة و تنعكس في تقليل الجهد و الوقت.
شملت المحطات التاريخ المرضى و الكشف السريري و الفحوصات المخبرية و الإشعاعية و الأجهزة الطبية و الأخلاقيات و قدرة الاتصال و علاقة المرضى و في كل مرة تتنوع و تتطور المحطات حيث شملت الفيديو و الشرائح مرضى حقيقيين و غير حقيقيين و بمشاركة الممتحنين الخارجيين و التقويم المستمر بكل أعضاء و التسيير و التطوير مما دفعنا لنشر هذه التجربة.
الخلاصة و الدروس المستقاة : أن التحضير و التنظيم الممتاز للاختبار السريري الموضوعي يحتاج في بادئ الأمر لكثير من الجهد و الوقت مع الجدية و الالتزام مما يزيد درجة الثبات و الوثوق و الذي لا شك في أنه يدفع العملية التعليمية و التدريبية للأمام.
أن الأوسكي بالرغم من أنه إضافة جيدة و معول عليها في التقييم إلا أن هناك بعض جوانب القصور و عدم الشمولية و صعوبة الجدارة التامة لذا لابد من دعمه بوسائل تقييم أخرى لضمان درجة الثبات المنشودة.
Abstract EN
Aims and objectives : In this article we aim to sharing and exchanging experiences at all levels of applying objective structured clinical examination (OSCE), to reduce trial and errors cost inefficiencies and to help and accelerate the acceptance the new comers to this assessment tool. Setting and Methods : The department of surgery, faculty of medicine, Umm-Alqura University Makkah adopting OSCE as an essential tool of assessment in fourth, fifth and sixth years, from the first batch of the students (1420H / 1999 / 2000).
It was first held in may 2000 (batch I- 21 students) and the numbers gradually increasing, till it reached 146 students in 2007 (batch 7).
During these seven years experience of running the examinations we have run till now more than twenty five consecutive OSCE examinations, and sometime quadruplicated, which have been continuously evaluated and reviewed with the help of our external examiners.
We are always refining the process and content with some development of new and innovative implementation, the department has improved the process and contents of OSCE. Lessons learned and conclusions : A well-organized OSCE requires significant investment in terms of planning, resources and enthusiasm, but can have potentially beneficial impacts on the assessments tools especially in big number of candidates.
However OSCE is a very good adjunct tool of assessment, and can be used in multicentre ways for big number of candidates, but also has its limitations and should be combined with other forms of assessment, which may be more valid and comprehensive to test competencies not easily tested within the OSCE format.
