مقدمة : تكثر حالات النزيف تحت الجافية في الأعمار المتقدمة و معظم الحالات تحدث لاصابات طفيفة و التي تنتنهي دائما بتجمع الدم تحت الجافية و الذي يتطلب التدخل الجراحي العاجل لتقليل الضغط على الدماغ و منع حدوث العواقب العصبية.
الهدف : الهدف من هذه الدراسة هو عكس نتائج عمليات ثقب الجمجمة المزدوج لعلاج تجمع الدم المزمن تحت الجافية.
المرضى و الطرق : أجريت هذه الدراسة على 70 مريض لديهم تجمع دم مزمن تحت الجافية و الذين ادخلوا مستشفى امدرمان التعليمي في الفترة ما بين نوفمبر 2004 و حتى نوفمبر 2006 جميع المرضى تمت مراجعتهم سريريا و تم تأكيد حالاتهم بفحص الأشعة المقطعية للدماغ.
و قد تمت إزالة تجمع الدم جراحيا عن طريق ثقب الجمجمة المزدوج.
النتائج : تضمنت الدراسة 70 مريضا (50 ذكر و 20 أنثى) متوسط أعمارهم 69 سنة الأشعة المقطعية للدماغ أظهرت تجمع الدم في جانب واحد لدى 60 % من المرضى، و في الجانبين لدى 40 % من المرضى، 1.8 % من المرضى اظهروا انفتاق في الدماغ.
ثقب الجمجمة المزدوج في جانب واحد أجري ل 60% من المرضى بينما أجريت في الجانبين ل 35.07 % من المرضى.
و في تتبع استباقي 87.1 % من المرضى حدث لهم شفاء كامل.
سبعة من المرضى حدثت لهم مضاعفات مثل استرواح الدماغ و إعادة التجمع مرة أخرى.
معدل الوفاة في هذه الدراسة كان 2.6 %.
الخلاصة : ثقب الجمجمة و تطبيق نظام الري و الصرف المغلق هي وسيلة فعالة تساعد في انحسار ما تبقى من تجمع الدم تحت الجافية مع القليل من حالات الانتكاس.
Abstract EN
Background : Chronic subdural hematoma (CSDH) is a common condition in late stages of life.
Most of the patients are subjected to minor trivial trauma which will end up with a collection of altered blood in the subdural space.
This necessitates immediate surgical drainage, with the aim to reduce the mass effect on the brain to alleviate the symptoms and to reverse the condition.
Objective : To report the experience and outcome of double burr-holes craniotomy in treatment of chronic subdural hematoma.
Patents and Methods : This study was conducted on 70 patients with CSDHs managed at Omdurman Teaching Hospital Sudan, during the period from November 2004 to November 2006..
All patients after clinical assessment and CT scan of the brain proved to have CSDH.
The hematoma was removed surgically by double burr-holes craniotomy, irrigation and close system drainage.
Results : The study included 70 patients, 50 males and 20 females.
The mean age was 69 years.
Computed tomography scan, showed unilateral collection in 60 % of patients and bilateral in 40 %, only 1.8 % showed conning of the brain.
Unilateral double burr-hole craniotomy was performed in 60 % of the patients, while bilateral was done in 35.7 %.
In the postoperative follow up 87.1 % of the patients showed uneventful recovery.
Seven patients developed complications in the form of pneumocephallus or postoperative recurrence.
The mortality rate was 2.9 %.
Conclusion : burr holes craniotomy, irrigation and close system drainage, is effective and favor rapid regression of residual subdural collection and associated with fewer recurrences.
