الأهداف : وصف الملامح السريرية، الحجمية و الخلوية للفص المعاكس في مجموعة من المرضى الذين يعانون من الدراق المتوطن المزمن في فص واحد من الغدة الدرقية.
المرضى و الطرق : تضمنت هذه الدراسة 60 مريض بالدراق المتوطن السريري في فص واحد من الغدة الدرقية جميع هؤلاء المرضى من غرب السودان.
اخضع جميع المرضى للجراحة و الاستكشاف المتزامن للفص الآخر.
النتائج : 50 (83 %) مريض من مجموع 60 مريض وجد لديهم المرض في فص واحد أما الفص الآخر لم توجد به أي عقديات و كان في حدود الحجم الطبيعي.
20 من المرضى أخذت منهم خزعة داخلية أثناء العملية و التي أظهرت أنسجة سليمة للغدة الدرقية في 16 منهم.
أوضحت الدراسة أن جميع المرضى الذين أجريت لهم إزالة الفص المريض قد حافظوا على مستويات طبيعية من هرمونات T3, T4, TSH.
الخلاصة : نقدم في هذه الدراسة مجموعة من المرضى الذين يعانون من الدراق المتواطن المزمن و الذين حينما حضروا للجراحة وجد لديهم أنواع متقدمة من المرض و التي تؤثر على فص واحد من الغدة الدرقية, مع وجود الفص المعاكس طبيعياً من حيث الشكل و الوظيفة.
Abstract EN
Objectives : To describe the clinical, volumetric and cytological features of the contra lateral lobe in a series of patients with long standing unilateral endemic goitre.
Patients and methods : This study included 60 patients from the west of Sudan with clinically detectable and long standing unilateral simple endemic goitre that required surgery, with the simultaneous exploration of the contra lateral lobe.
Results : Out of 60 patients with unilateral goitre, 50 (83 %) were found to have the disease on the ipsilateral lobe only (monolobar goitre).The contra lateral lobe in these 50 patients showed normal size with nodularity.
20 patients with monolobar disease had intra-operative fine needle aspiration cytology (FNAC), of which 16 patients showed normal thyroid tissue.
All patients with monolobar disease had lobectomy.
Post-operatively they continued to have normal blood levels of T3, T4 and TSH.
Conclusion : We report a series of patients with endemic goitre who had advanced forms of the disease that affect only one lobe in the presence of a structurally and functionally normal contra lateral lobe.
