المقدمة و الأهداف :لا يزال هنالك نقاش و اختلاف في الآراء نحو معالجة تحجر الزائدة الدودية (Appendicular Mass) و نحو توقيت التداخل الجراحي.
الهدف من هذه الدراسة هو مراجعة و تقييم طرق معالجة تحجر الزائدة الدودية في مستشفى الخرطوم التعليمي, السودان.
المرض و الطرق : شملت الدراسة 280 مريض, تم تحليل البيانات الشخصية و السريرية لهم, و ذلك خلال الفترة يناير 2000 – ديسمبر 2008م.
النتائج : 104 مريض (37.5%) كانوا من الثلاثينات من العمر, 204 مريض (72.9%) اشتكوا من ألم في البطن لمدة 5 أيام, 136 مريض (48.6%) كانت درجة الحرارة عندهم أكثر من 37.5م.
المعالجة التحفظية كانت ناجحة في 156 مريض (55.7%), بينما 25 مريض (8.9%) أجريت لهم جراحة عالجة لإزالة الزائدة الدودية.
و هنالك 28 مريض (10%) حضروا للمتابعة لكنهم رفضوا العملية الجراحة.
اتضح وجود قيلة مخاطية (Mucocele) في الزائدة الدودية عند مريض واحد, و سرطان الأعور (Caecum) في مريض آخر.
الجراحة العاجلة كانت صعبة في 8 مرضى حيث لم يتم إزالة الزائدة في مريض واحد و حدث ناسور برازي (Faecal Fistula) في إثنين من المرضى.
الخلاصة : طريقة العلاج التحقظي للزائدة الدودية المتحضرة تعتبر مأمونة, لكن سرطان الأعور قد يعمم تشخيصه (Missed), و في المقابل فإن الجراحة العاجلة في حالات قليلة قد أدت إلى حدوث ناسور برازي.
Abstract EN
Background: Management and timing of surgery for appendicle mass is controversial. Objective: To audit the management of appendicle mass in Khartoum Teaching Hospital. Methods: Analysis of demographic and clinical data of 280 patients in the period Jan 2000 through Dec 2006. Results: Out of 280 patients 104(37.5%) were in the third decade.
204(72.9%) had pain for more than five days and 136(48.6%) had temperature >37.5oC.
Conservative management was successful in 156(55.7%) patients.
25(8.9%) patients had emergency surgery.
28(10%) patients came for follow up but refused surgery.
Mucocele of the appendix and carcinoma of the caecum were found each in one patient.
Emergency surgery was difficult in eight patients with failure to remove the appendix in one of them and faecal fistula developed in tow. Conclusion: The conservative method is safe.
However, cancer caecum may be missed.
In contrast emergency surgery led to faecal fistula in two patients.
