أجريت دراسة ضبطية على (65) من الرضع و الأطفال المصابين بالخمج و الذين تم إدخالهم إلى وحدات الأطفال في مستشفى البصرة للولادة و الأطفال للفترة من الأول من أيلول 2002 و لغاية أواخر شهر شباط 2003.
تراوحت أعمارهم من شهر واحد-9 سنوات (37 طفلا كانوا من الذكور و 28 من الإناث).
خمس و سبعون طفلا (43 من الذكور و 32 من الإناث) ممن راجعوا قسم العيادة الخارجية لغرض التلقيحات أو أمراض بسيطة مثل الزكام تم اختيارهم عشوائيا كمجموعة ضابطة.
تم أخذ تاريخ المرض بالإضافة إلى إجراء الفحص السريري لجميع الأطفال الذين تضمنتهم الدراسة بالإضافة إلى التحاليل التالية : - مستوى خضاب الدم، قياس الدم البيضاء، مستوى الفسفور، الكالسيوم و الألبومين في الدم، انخفاض مستوى الفسفور في الدم وجد عند 20 % من المرضى المصابين بالخمج، بينت الدراسة إن متوسط مستويات الفسفور، الكالسيوم، الألبومين و خضاب الدم هي أقل عند المرضى مقارنة بالمجموعة الضابطة و عدد كريات الدم البيضاء هي أعلى عن المرضى المصابين بالخمج مقارنة بالمجموعة الضابطة (علما أن هذه الفروقات معتدة إحصائيا).
لم تثبت الدراسة وجود معتد إحصائيا في نسبة الفسفور في الدم مقارنة بعمر الطفل، الجنس، فترة المرض أو نوع الخمج.
يمكن الاستنتاج من هذه الدراسة أن نسبة مستوى الفسفور المنخفض عالية بين الأطفال و الرضع المصابين بالخمج و إن هناك حاجة لدراسات أخرى لتوضيح أسباب انخفاض مستوى الفسفور و دور العاج المبكر بالفسفور لهؤلاء المرضى.
Abstract EN
A case-control study was carried out on 65 infants and children with infectious diseases admitted to pediatric wards of Basrah Maternity and Child Hospital from the f of September 2002 till the end of February 2003.
Their ages ranged from 1 month-9 years, 37weremales and 28 were females.
Seventy-five children (43 males and 32 females) visiting the outpatient department for vaccination or for minor illnesses like common cold were randomly selected as controls.
For all children included a complete history and thorough physical examination were done and the following investigations were carried out ; hemoglobin level, white blood cell count, serum phosphorus, calcium and albumin.
Hypophosphatemia was detected in 20 % of patients with infections.
The study has revealed that the mean serum phosphorus, calcium, albumin and hemoglobin levels were significantly lower while the mean white blood cell count was significantly higher in patients compared to controls.
There was no significant difference in mean serum phosphorus level in relation to age of patients, sex, duration of illness or the type o infection.
From this study it is concluded that hypophosphatemia is high among infants and children with infections and further studies are needed to explain the causes of hypophosphatemia and the role of early phosphate therapy In such patients.
