Purpose : evaluating the role of insulin, IGF-I and IGFBP-3 in the development of premenopausal breast cancer compared to benign breast conditions (namely cystic disease of the breast) and normal control cases.
In addition, re evaluating the levels of the studied parameters post operatively in breast cancer cases (one week after the operation).
Patients and Methods : three groups were studied, 40 patients with different stages of cancer breast all premenopausal, 25 patients with Cystic disease of the breast and 25 normal females with normal breast.
All cases were tested for FBS, Serum Insulin, Serum IGF- I and SerumIGFBP-3.
14 breast cancer cases (randomly selected) were re tested for the previous parameters one week post operatively.
Results : Serum insulin level, serum IGF- I and serum IGFBP-3 of the breast cancer cases were found to be significantly higher than that of either cystic disease cases or the normal control group.
But mean FBS level of breast cancer cases showed insignificant difference with that of cystic disease case or that of normal control cases.
Moreover, significant drop was recorded in the serum level of IGF-1 and IGFBP-3 post operatively.
Conclusion : Hyperinsulinemia with insulin resistance (which is independent of obesity) could present an early step in the pathogenesis of breast cancer in premenopausal women
